2型糖尿病胃镜检查131例临床分析

摘　　要：目的探讨2型糖尿病患者胃镜下的特点、病理特征、幽门螺杆菌感染情况以及糖尿病病程、糖化血红蛋白对胃病的影响。方法选取行胃镜及病理检查的2型糖尿病患者131例,并与同时期非糖尿病患者进行比较,并对糖尿病患者行糖化血红蛋白检测,观察血糖对胃病的影响。结果糖尿病组胃炎合并十二指肠病变及糜烂性胃炎检出率均高于对照组(P均<0.05),糖尿病组幽门螺杆菌感染率47.3%(62/131),对照组幽门螺杆菌感染率33.0%(37/112)(P<0.05),糖尿病Hp阴性组中胃炎合并反流性食管炎检出率高于对照组(P<0.05)。糖尿病病程及糖化血红蛋白对胃病无明显影响。结论 2型糖尿病患者幽门螺杆菌感染率高,对胃炎合并十二指肠病变及糜烂性胃炎检出率高,存在癌前病变的组织学特征的患者应保存高度的临床警惕性,定期进行内镜随访检查、组织病理学检测。Objective To investigate gastroscope clinical characteristics,pathological characteristics and helicobacter pylori（Hp）infection status of patients with type 2diabetes mellitus（T2DM）.Besides,to explore the effect of diabetes course and HbA1 c on gastric disease.Methods We selected 131 patients with T2 DM who received gastroscopy and pathological examination,compared with non diabetic patients into the same period,to study gastroscopic clinical and pathological characteristics.Through HbA1 c test,we observed the effects of glucose on the stomach disease.Results In diabetes group,the rate of gastritis with duodenal lesions and erosive gastritis was higher than that of control group（P〈0.05）.Hp infection rate in diabetes group was 47.3%（62/131）,helicobacter pylori infection rate in control group was 30.0%（37/112）（P〈0.05）.In Hp negative diabetes group,the rate of gastritis with reflux esophagitis was higher than that of control group（P〈0.05）.Diabetes course and HbA1 c had no obvious effect on gastric disease.Conclusion In patients with T2 DM,Hp infection rate was higher,the rate of gastritis with duodenal lesions and erosive gastritis was higher,patients with histological features of precancerous lesions should maintain a high clinical vigilance.It is necessary to do endoscopic follow-up check and histopathological examination in a regular stage.
