摘  要:目的:采用自身对照方法,观察外用重组人表皮细胞生长因子(rh-EGF)凝胶联合磺胺嘧啶银霜(SD-Ag)对兔深Ⅱ°烫伤创面愈合的影响,探索rh-EGF凝胶联合SD-Ag联合用药在促进创面愈合中的作用机制。方法:选取20只健康成年新西兰大白兔,于脊柱两侧由前至后制作3对共6个直径3.0 cm圆形深Ⅱ°烫伤创面,由前至后定为A组(联合用药组)、B组(rhEGF凝胶组)、C组(SD-Ag组),创面分别涂抹2~3 mm厚药物;分别于烫伤后4、7、10、14 d观察创面愈合情况,计算创面愈合率、检测微血管密度。结果:各组创面随时间延长而逐渐缩小,各时间点A组创面愈合率均高于B、C组(P〈0.05);烫伤后4~7 d细菌数量开始减少,14 d时细菌大部分被机体自身清除,A、C组与B组差异有统计学意义(P〈0.05),而A、C组间差异无统计学意义(P〉0.05);微血管密度值(MVD)A、B组显著增加,其中A组呈高度表达(P〈0.05)。结论:rh-EGF凝胶联合SD-Ag联合应用于创面,比单独用药效果明显,在提供所需湿润环境同时也能抑制炎症侵润,保护创面并对控制创面感染有一定预防性作用。Objective: To observe the therapeutic effect of the recombinant human epidermal growth factor(rh-EGF) and sulfadiazine silver(SD-Ag) on deep second degree burn wound and to explore its molecular mechanism in rabbits. Methods: Twenty healthy adult New Zealand white rabbits were randomly selected, and 3pairs of 3 cm circular deep II degree burn wounds symmetrically distributed on both sides of the spine were created on each rabbit. From head to tail, each pair of wound were designated as group A(treated with the combination of rh-EGF gel and SD-Ag), group B(treated with rh-EGF gel alone); and group C(treated with SD-Ag alone). The wounds were covered sequentially with 2 ~ 3 mm thick drug, sterile dressing, and vaseline gauze, which was replaced once a day. Visual assessment of wound healing, wound healing rate, and microvessel density were evaluated on day 4, 7, 10, and 14 post-injury by immunohistochemical staining. Results: With time,the size of each wound was gradually reduced, and the wound healing rate at each time point in combination treatment group were higher than in the other two single treatment groups. Bacterial load began to decrease on day4 to 7 post-burn injury. The most bacterial were cleared on day 14. The difference between group A and B and between group C and B were statistically significant, whereas the difference between group A and C did not exist and the value of MVD was high in both group A and group B, with group A higher than group B. Conclusion:The therapeutic effect of combined application of rh-EGF gel and SD-Ag in the wound healing is better than either medication used alone, which provides the necessary moist environment and inhibits the inflammatory infiltration into the wound.
