持续股神经阻滞下人工全膝关节置换术后早期功能恢复39例临床疗效分析

摘  要:[背景]观察持续股神经阻滞(CFNB)下人工全膝关节置换术(TKA)后早期功能恢复的临床疗效.[病例报告]选择2012年12月至2014年3月间行TKA后采用CFNB进行膝关节早期功能恢复锻炼的39例作为实验组,采用静脉镇痛泵进行膝关节早期功能恢复锻炼的33例作为对照组,比较观察术后24h及72h时两组膝关节屈伸角度及VAS视觉疼痛评分.结果见,术后24h实验组膝关节平均屈伸角度明显大于对照组(P<0.05);术后72h实验组膝关节平均屈伸角度显著大于对照组(P<0.05),VAS评分明显低于对照组(P<0.05).[讨论]CFNB可有效缓解TKA患者术后疼痛症状,提高TKA患者术后早期功能恢复的效果.BACKGROUND To explore the effect of early rehabilitation of total knee replacement(TKA) with continuous femoral nerve block. CASE REPORTS 39 TKA patients with continuous femoral nerve block were selected as experimental group, comparing with 33 other TKA patients with venous analgetic pump as control group between December 2012 and March 2014. The flexion angle was observed and visual analog pain scores (VAS) were evaluated at 24 and 72 hours after the surgery. The results showed that the flexion angle of experimental group was better than control group at 24 hours after the surgery (P〈0.05), the flexion angle and VAS scores of experimental group were both better than those of control group at 72 hours after the surgery(P〈0.05). DISCUSSION Continuous femoral nerve block could obviously relieve pain and enhance the early rehabilitation after TKA.
