CT引导下蝶腭神经节脉冲射频治疗难治性丛集性头痛的疗效观察

摘　　要：目的：报道CT引导下蝶腭神经节脉冲射频治疗（pulsed radiofrequency treatment,PRFT）20例难治性丛集性头痛（cluster headache,CH）的效果。方法：20例难治性丛集性头痛,17例阵发性丛集性头痛（episodic cluster headache,ECH）,3例慢性丛集性头痛（chronic cluster headache,CCH）,患者于CT引导下行蝶腭神经节脉冲射频治疗,记录疗效和副作用。结果：15例（88%）ECH和1例（33%）CCH患者在接受治疗后平均5.5（1～30）天疼痛完全缓解,2例（12%）ECH、2例（67%）CCH患者疼痛无缓解。平均随访时间为27（12～45）月。入组所有患者无治疗相关的不良反应和并发症。结论：本研究表明CT引导下脉冲射频治疗蝶腭神经节在难治性ECH患者可迅速、有效、安全促进丛集期的缓解,有望成为保守治疗无效时的一种选择。Objective： To evaluate the efficacy and safety of a non-ablative computerized tomography （CT）-guided pulsed radiofrequency treatment of sphenopalatine ganglion in patients with refractory cluster headaches. Methods： Twenty consecutive cluster headache patients （17 episodic cluster headaches and 3 chronic cluster headaches） was treated with pulsed radiofrequency of sphenopalatine ganglion under CT guidance. Results： The average follow-up time was 27 months （ranged 12 - 45 months）. The headache of 15 episodic cluster headache patients and 1 chronic cluster headache patients were completely relieved within an average of 5.5 d （ranged 1 - 30 d） following the treatment. Two episodic cluster headache patients and 2 chronic cluster headache patients showed no pain relief following the treatment. All patients enrolled in this study did not show treatment-related side effects or complications. Conclusion： Our data show that the pain of patients with refractory episodic cluster headaches was quickly, effectively and safely relieved from the cluster period after CT-guided pulsed radiofrequency treatment of sphenopalatine ganglion, suggesting that it may be a therapeutic option if conservative treatments fail.
