用两种经皮气管切开术对重症患者进行抢救的效果对比

摘　　要：目的：对比分析用经皮导丝扩张钳气管切开术（GWDF）和经皮旋转气管切开术（Percutwist）对重症患者进行抢救的临床效果。方法：对2012年3月～2015年6月期间我院急诊科、重症监护室收治的156例重症患者的临床资料进行回顾性研究。我们根据手术方式的不同将这156例患者分为对照组（58例）和研究组（98例）。我们对对照组患者使用经皮旋转气管切开术进行抢救，对研究组患者使用经皮导丝扩张钳气管切开术进行抢救。治疗结束后，比较两组患者手术的用时、手术切口的长度、手术的难易程度及术后并发症的发生率。结果：研究组患者手术的用时明显短于对照组患者，其手术的难度明显低于对照组患者，其并发症的发生率明显低于对照组患者，二者相比差异有统计学意义（P ＜0.05）。两组患者手术切口的长度相比差异无统计学意义（P ＞0.05）。结论：与进行经皮旋转气管切开术相比，用经皮导丝扩张钳气管切开术对重症患者进行抢救具有手术时间短、手术难度低、患者术后并发症的发生率低等优点。因此，经皮导丝扩张钳气管切开术可作为对重症患者进行抢救的优选手术。objective： to study the percutaneous guide wire dilating forceps tracheotomy （Poaex method） and percutaneous tracheotomy rotating （Percutwist method） the clinical effect of two kinds of operation ways. Methods： selected from January 2012 - February 2013 patients in the intensive care unit 62 cases as the research object, combined with surgery in patients with intend group, the observation group （31 cases） in patients with percutaneous guide wire dilating forceps tracheotomy, the control group （31 cases） in patients with implementation of percutaneous tracheotomy spin, statistics of two groups of patients with operation time, degree of difficulty degree of incision, operation and postoperative complications and other specific indicators. Results： the observation group of patients operation time （5.1 ＋ 0.8） min, the control group （6.5 ＋ 1.1） min; Observation group surgery I in 15 cases, 2 degrees in 16 cases, control group surgery I in 22 cases, 2 degrees 9 cases; The complication rate of observation group of patients 11.2%, control group patients complication rates 24.1%, significant difference （P 〈 0.05）. Conclusion： Poaex method in Percutwist method of＆nbsp;shorter operation time and lower incidence of complications, but the operation difficulty is higher, the doctor should be combined with the actual situation to choose.
