儿童腹型过敏性紫癜外周血淋巴细胞临床分析

摘　　要：目的探讨腹型过敏性紫癜(Henoch-Schonlein Purpura,HSP)患儿外周血淋巴细胞亚群的变化特点及临床意义。方法流式细胞术检测579例过敏性紫癜患儿外周血T淋巴细胞(CD3+、CD4+、CD8+、CD4/CD8)、B细胞(CD19+)以及NK细胞(CD16+、CD56+)水平,比较腹型与非腹型过敏性紫癜患儿外周血淋巴细胞亚群的差异。结果与非腹型HSP患儿比较,腹型HSP患儿的发病年龄较低,性别差异无统计学意义,CD3+(t=-11.435,P<0.001)、CD4+(t=-7.840,P<0.001)和CD8+(t=-5.186,P<0.001),CD4/CD8差异无统计学意义(P>0.05),B细胞水平显著上升(t=10.106,P<0.001),NK细胞水平上升(t=2.825,P=0.005)。结论过敏性紫癜患儿存在外周血T淋巴细胞免疫功能紊乱,T淋巴细胞亚群含量的变化尤其是B淋巴细胞的升高,体液免疫增强,有助于腹型和非腹型HSP的鉴别。Objective To investigate the characteristics of peripheral blood lymphocytes in pediatric allergic purpura（Henoch-Sch¨onlein Purpura,HSP） patients with or without abdomen involvement,and provide reference for diagnosis and treatment of children with HSP. Methods Retrospective analysis was conducted on the amount of peripheral blood lymphocyte subsets in 579 children with HSP. Flow cytometry were used to detect the T cell（CD3＋,CD4＋,CD8＋）,B cell（CD19＋） and NK cell（CD16＋CD56＋）levels in peripheral blood. The differences between HSP patients with and without abdomen involvement were analyzed comparatively.Results Compared with HSP patients without abdomen involvement,HSP patients with abdomen involvement were of younger age,and showed significantly increased CD3＋（t =-7.077,P 0.001）,CD4＋（t =-3.558,P 0.001）,CD8＋（t =-5.326,P 0.001） T cells and lymphocytotic B cells（t = 7.181,P 0.001）. There was no significant difference in CD4/CD8 between these two groups.Conclusion Peripheral T cell dysfunction exists in HSP patients. The amount of T cell subtypes can help to determine whether or not there is abdomen injure in HSP patient.
