摘 要:目的讨论时空关联成像联合超声断层显像技术(STIC-TUI)在产前评价诊断胎儿室间隔缺损的应用价值。方法选取产前超声筛查中可疑先天性心脏病(CHD)的胎儿79例行产前二维超声心动图检查,并应用时空关联成像技术(STIC)获取胎儿心脏容积数据、TUI技术进行脱机分析,对所有可疑病例产前超声定期随诊,产后新生儿超声心动图,磁共振成像(MRI),多排螺旋CT及引产后尸检证实,对比TUI技术对不同类型的VSD的诊断差异。结果 79例可疑CHD胎儿病例中48例胎儿有VSD,其中产前经常规2DE诊断34例,符合率71%,经TUI诊断44例,符合率为92%,其中直径≤4 mm的VSD常规2DE诊断模式符合率为69%,TUI模式诊断符合率为100%,直径>4 mm的VSD常规2DE诊断模式符合率为94%,TUI模式诊断符合为100%。常规2DE漏斗部VSD符合率100%;膜周部VSD符合率86%;肌部VSD符合率80%。TUI模式漏斗部VSD符合率100%;膜周部VSD符合率88%;肌部VSD符合率93%。对于小型室间隔缺损及肌部室间隔缺损TUI-STIC较2DE具有统计学差异(P<0.05)。结论在诊断胎儿VSD方面TUI模式相对于常规二维超声心动图在小型室间隔缺损,肌部室间隔缺损诊断方面具有一定的优势。Objective To discuss the diagnostic value of spatio-temporal correlation imaging combined with ultrasound imaging (STIC-TUI) in prenatal diagnosis of fetal application value of ventricular septal defect. Methods Prenatal ultrasound screening was underwent in 79 cases with suspected congenital heart disease (CHD), the fetal heart volume data were obtained by using temporal spatial correlation imaging (STIC), TUI offline analysis of technology, prenatal ultrasound for all suspected cases while postpartum neonatal echocardiography, MRI, multiple spiral CT and postmortem examination after induction of labor were adopted. Contrast TUI technology for different types of VSD diagnostic differences. Results Forty-eight cases were diagnosed fetal VSD. Among them, 34 cases were diagnosed by prenatal regular 2DE, with coincidence rate of 71%, 44 cases diagnosed by TUI, compliance rate was 92%, with a diameter less than 4ram VSD model in which the conventional 2DE diagnosis rate was 69%, the diagnostic accuracy of TUI model was 100%. The coincidence rate of the conventional 2DE diagnostic model of VSD with the diameter of 〉4 mm was 94%, TUI pattern diagnosis was 100%. The VSD compliance rate of conventional 2DE was 100%. The VSD compliance rate of the membrane was 86%. Muscle VSD compliance rate was 80%. TUI model of the funnel section VSD compliance rate of 100%. The VSD compliance rate of the membrane was 88%. Muscle VSD compliance rate was 93%. There was statistical difference (P〈0.05) between TUI-STIC and 2DE in small ventricular septal defect and muscular ventricular septal defect. Conclusion Compared with conventional two-dimensional echocardiography in diagnosing fetal VSD, TUI model has certain advantages in the diagnosis of ventricnlar septal defects.
