摘 要:目的 探讨低场强磁共振成像(MRI)在无骨折脱位型颈髓损伤临床诊治中的应用。方法 收集19例无骨折脱位的颈髓损伤患者的MRI表现、临床资料,总结MRI检查在无骨折脱位型脊髓损伤诊治中的重要性。结果 本组病例中X线、CT检查均未见骨折脱位,有颈椎间盘突出及椎体后缘增生、后纵韧带骨化,黄韧带肥厚、小关节突增生所致椎管狭窄,颈髓前、后受压,其中:颈髓前方受压14例,颈髓后方受压5例,颈髓前后同时受压6例;颈髓损伤平面对应椎管狭窄、脊髓受压平面:颈3-4水平11例,颈4-5水平5例,颈5-6水平2例,有1例颈髓损伤平面不同于脊髓受压平面。MRI信号表现:脊髓肿胀、脊髓水肿、脊髓挫伤。结论 MRI检查是诊断无骨折脱位的脊髓损伤的最准确、最有效检查方法,为临床医师评估预后、制定合理的治疗方案,提供可靠依据。Objective To explore the low field MRI in the type cervical cord injury without fracture dislocation the application of the disagonsis and treatment. Methods Ninteen csaes of patients with cereical spinal injury without fracture dislocation MRI manifestation, clinical data, summarize MRI in the diagnosis and treatment of the spinal cord injury without fracture dislocation type. Results Incidence of X-ray and CT examination were not seen. Fracture dislocation of cervical intervertebral disc hemination and vertebral hyperplasia of trailing edge and longitudinal ligament ossification, yellow ligament hypertrophy, hyperplasis of the small joints caused by spinal canal stenosis, carotid pulp before and after compression. Among them there were 14 cases with cervical spinal anterior compression. Comperssion of cervical spinal rear in 5 cases, 6 cases before and after cervical spinal compression at the same time; Cervical spinal injury plane corresponding stennsis, spinal cord compression; neck 3 or 4 levels in 11 cases; neck 4-5 levels in the 5 cases; neck 5 to 6 level in 2 cases; 1 cases of the cervical spinal injury plane was different spinal cord amomg them; 14 cases cervical spinal anterior compression, compression of the spinal rear in 5 cases; 6 cases before and after cervical spinal compression at the same time. Cervical spinal injury plane correspongdings stenosis, spinal cord compression, neck 3 or 4 levels in 11 cases, neck 4-5 levels in 5 cases, neck 5 to 6 level in 2 csese,1 cases of cervical spinal injury plane is different from the spinal cord compeerssion. MR signal was showed as swelling in the spinal cord ,spinal cord edema,spinal cord contusion. Conclusion MRI is the most accurate diagonsis of spinal cord injury without fracture dislocation, the most effective inspection method ,for clinclal physiciants to evaluate prognosis, and reasonsble treatment,to provide reliable basis.
