摘  要:目的探讨甲型H1N1流感肺炎的胸部影像学征象及治疗前后动态变化。方法对21例确诊甲型H1N1流感患者进行多层螺旋CT(MSCT)扫描(含HRCT)回顾性分析、观察病变的影像学特点及治疗前后相关动态变化。结果甲型H1N1流感胸部CT表现及动态变化特点:1磨玻璃密度影和肺实变密度影,也可为间质性病变、小结节等,主要沿支气管血管中心肺区和(或)胸膜下区分布。2病变以两肺散在分布为主,单侧分布以下肺分布显著。3发展规律:早期,多为肺内局灶性实变影;进展期,病变融合呈散在多发的实变影,肺门部融合多;恢复期,以肺间质改变为主。结论甲型H1N1流感肺炎的胸部CT表现有一定特征性,影像学检查与诊断仍是甲型H1N1流感肺炎的临床诊断、疗效评价、预后分析以及鉴别诊断的重要方法。Objective To discus H1N1 influenza pneumonia in chest imaging appearance and the dynamic changes before and after treatment. Methods The MSCT data of 21 cases confirmed H1N1 influenza patients (ineluding HRCT) were retrospectively analyzed, then observe the imaging features betore and after treal,nenl-related change dynamically. Results The features of chest CT findings and dynamic changes were as follows.①Groundglass opacity and pulmonary consolidation density, but also for interstitial lung disease, small nodules, Mainly along the bronchial pulmonary vascular center area and (or) subpleural distribulion area.②Lungs lesions scattered in the main distribution the following unilateral lung distribution significantly. ③The regulation of development: early, mostly focal puhnonary opacities, advanced lesions were scattered in muhiple integration of opacities, hilar fusion of muhi; recovery to alter pulmonary interstitial dominated. Conclusion Influenza HINI influenza pneumonia in chest CT manifestations has certain characteristic, and the imaging examination and diagnosis is still the influenza a (HINI) pneumonia clinical diagnosis, curative effect evaluation prognosis and differential diagnosis.
