脑脊液置换联合鞘内注药治疗结核性脑膜炎的效果分析

摘　　要：目的探究脑脊液置换联合鞘内注药治疗结核性脑膜炎的临床疗效。方法选取2013年1月~2016年1月我院收治的结核性脑膜炎患者共81例,将其随机分为对照组（n=40）与观察组（n=41）,对照组患者接受常规四联抗结核治疗,观察组在常规四联抗结核治疗基础上给予脑脊液置换联合鞘内注药。比较两组患者结核性脑膜炎治疗效果、病程等指标。结果观察组治疗总有效率为92.7%,明显高于对照组的75.0%（P〈0.05）。观察组发热、头痛、脑膜刺激征及颅内高压消失时间均明显短于对照组（P〈0.05）。治疗后,观察组脑脊液白细胞计数、蛋白质含量明显低于对照组（P〈0.05）。两组患者并发症发生率比较,差异无统计学意义（P〉0.05）。结论脑脊液置换联合鞘内注药治疗结核性脑膜炎有较好的临床疗效,能有效缩短病程,安全可靠,值得临床推广应用。Objective To explore the clinical efficacy of cerebrospinal fluid（CSF） replacement combined with intrathecal injection in the treatment of tuberculous meningitis.Methods 81 patients with tuberculous meningitis were selected from January 2014 to January 2016 in our hospital,all patients were randomly assigned to the control group（n=40） and the observation group（n =41）.The control group received conventional antituberculosis treatment,the observation group treated with CSF replacement combined with intrathecal injection on the basis of conventional treatment.The tuberculosis meningitis treatment effect,course of disease and other indicators were compared of two groups.Results The total effective rate of the observation group was 92.7%,which was significantly higher than 75.0% of the control group（P 0.05）.The disappearance time of fever,headache,dural irritation and intracranial hypertension in the observation group was significantly shorter than that in the control group respectively（P〈0.05）.After treatment,the white blood cell count and protein content of the observation group was significantly lower than that of the control group（P〈0.05）.There was no significant difference in the incidence of complications between the two groups（P〉0.05）.Conclusion CSF replacement combined with intrathecal injection in the treatment of tuberculous meningitis has a good clinical effect,can effectively shorten the course of disease,safe and reliable,worthy of clinical application.
