皮质引流静脉保护在重型闭合性颅脑损伤神经功能恢复中的意义

摘　　要：目的探讨皮质引流静脉保护在重型闭合性颅脑损伤神经功能恢复中的意义。方法选取本院2012年1月~2014年8月因重型闭合性颅脑损伤行手术治疗的40例患者作为研究对象,根据是否术中行皮质引流静脉保护分为研究组和对照组,各20例。比较两组的术侧水肿体积、颅内压监测值、并发症发生率（瘫痪、精神异常、癫痫发作、意识障碍）以及GOS评分。结果实验组术后3 d的术侧水肿体积、颅内压平均值显著低于对照组,差异有统计学意义（P〈0.05）。研究组术后的瘫痪、意识障碍发生率显著低于对照组,GOS评分显著高于对照组,差异有统计学意义（P〈0.05）。结论在重型闭合性颅脑损伤手术中骨窗边缘下塑造血管保护通道能够保护皮质引流静脉受压,减少静脉回流障碍,对减少并发症、改善患者预后有重要意义。Objective To explore the significance of cortical venous drainage protection in the recovery of neurological function in patients with severe closed craniocerebral injury.Methods 40 severe closed craniocerebral injury patients underwent surgery from January 2012 to August 2014 in our hospital were selected and divided into the experience group and the control group according to whether or not the cortical venous drainage protection,20 cases in each group.Postoperative lateral edema volume,intracranial pressure monitoring value,the incidence rate of complication（paralysis,mental disorders,seizures,disturbance of consciousness） and GOS score in the two groups was compared.Results The volume of postoperative lateral edema and the average value of intracranial pressure after operation at 3 days in the experience group was lower than that in the control group,with significant difference（P〈0.05）.The incidence rate of paralysis,disturbance of consciousness in the experience group was lower than that in the control group,the score of GOS in the experience group was higher than that in the control group,with significant difference（P〈 0.05）.Conclusion Bone window edge shaping vascular protection channel in severe closed craniocerebral injury can protect the cortex venous drainage compression,reduce venous return obstacles and has great meaning in reducing complication and improving prognosis of patients.
