摘 要:目的 研究腰神经根损伤后 ,神经肌肉接头部神经再支配过程中的形态变化。方法 采用免疫组织化学方法和激光共聚焦 (Confocallaserscaningmicroscopy)技术 ,使用多克隆蛋白基因产物 (PGP9 5)作为神经原标记 ,观察了大鼠L5神经根受压后 10、2 0、30和 6 0d比目鱼肌神经肌肉接头部末梢神经变性及再生修复过程 ,运用α bungarotoxin(α BTX)萤光结合剂显示运动终板 ,NIH技术测定末梢神经与运动终板重叠面积。结果 肌肉失神经支配后 10天 ,神经肌肉接头部末梢神经大幅度消失 ,运动终板形态结构紊乱 ,随着神经再支配的进行 ,通过末梢外突起的延伸并与邻近的运动终板相连 (终板间连接 ) ,在神经肌肉接合部形成一个多神经支配复杂的制作过程。结论 神经再生修复过程中 ,神经纤维与后突触受体区域的变化有一个时空关系 ,同时PGP9 5免疫组织化学标记 ,结合激光共聚焦在正常与再生的神经肌肉接合部 ,能清晰地显示精致的神经末梢 ,因此 ,PGP9 5抗体可用于研究各种不同情况下肌肉神经支配。Objective To study the morphological changes in neuromuscular junction after L 5 nerve root compressed in rat Methods Using immunohistochemistry and confocal laser scanning techniques and polychonal antibody protein gene product 9 5 as neuronal marker, we observed the process of degenerating and regenerating neuromuscular junctions in the soleus muscle of the rat 10, 20, 30 and 60 d after L 5 nerve root compressed The motor endplat es were visualized with fluorescein conjugated α bungarotoxin (α-BTX) The overlap area of terminal and endplates was measured with NIH image technique Results 10 d after L 5 nerve root injury resulted in disappearance o f axon terminal and disorder of motor endplates, reinnervation resulted in an e xtraterminal sprout extending to an adjacent endplate (endplate to endplate co nnections) and forming fine processes that consisted of terminal branch and mult iple nerves Conclusion There is spatiotemporal relationship of t hese axonal changes to that of the postsynaptic receptor region during muscle reinnervation Confocal laser scanning microscopy of PGP 9 5 immunocytochemistry demonstrates the three dimensional organization of elaborate axon terminal at high resolution in the normal and regenerating junctions Using this technique, extremely fine processes of axonal terminal are identifiable at the earliest stage of reinnervation Thus,PGP 9 5 antibodies may serve as a tool for investigation of muscle innervation,reinnervation and nerve regeneration in different conditions
