朝鲜族和汉族产妇无手术指征剖宫产原因分析

摘　　要：[目的]分析延边地区朝鲜族和汉族产妇无手术指征剖宫产的原因.[方法]选择2014年1月至2014年6月间在延边大学附属医院行无手术指征剖宫产术的产妇338例进行分析,其中朝鲜族154例,汉族184例.采用无指征剖宫产原因调查量表,由专人对两组产妇进行一对一专访,完成表格内容并进行比较.[结果]338例无手术指征的剖宫产产妇中原因为择日生产126例(37.28%)、对自然分娩感到恐惧60例(17.75%)、不能忍受自然分娩疼痛46例(13.61%)、高龄初产42例(12.43%),其余为剖宫产安全、方便、无痛苦、珍贵儿、剖宫产对孩子有利、剖宫产对日后性生活有利等原因.汉族和朝鲜族产妇无指征剖宫产原因比较结果显示,朝鲜族择日生产、认为剖宫产对孩子有利为原因者明显多于汉族(P<0.05);汉族不能忍受自然分娩疼痛、认为剖宫产安全无痛苦、珍贵儿等为原因者明显多于朝鲜族(P<0.05).[结论]择日生产、认为剖宫产对孩子有利、不能忍受自然分娩疼痛、剖宫产安全无痛苦及珍贵儿是延边地区产妇无手术指征剖宫产主要原因.OBJECTIVE To analyze the reasons of cesarean section without operation indications in puerperae of the Korean-Chinese nationality and Han nationality in Yanbian area. METHODS A total of 338 puerperae were undergone cesarean section without operation indication between January and June 2014 in affiliated hospital of Yanbian university, with 154 Korean-Chinese nationality and 184 Han nationality. Questionnaire scale of cesarean section reasons was used, and the puerperae in the two groups were interviewed one-to-one by professionals to finish and compare the questionnaires. RESULTS Choosing an auspicious day for delivery was the most common reason that accounted for 37. 28% （126 cases）, fear, intolerance to natural childbirth pain and elderly primiparae accounted for 17.75% （60 cases）, 13.61% （46 cases） and 12.43% （42 cases） respectively. Safety, convenience, less pain, benefit for fetus and future sexual life of cesarean section and precious fetus were the other reasons. After comparing the reasons of cesarean section between the Korean-Chinese and Han nationality, the results showed that the rate of choosing an auspicious day for delivery and benefit for fetus of cesarean section in the Korean-Chinese nationality were significantly higher than that in the Han nationality （P〈 0.05）, while the rate of intolerance to natural childbirth pain, safety and less pain of cesarean section and precious fetus in the Han nationality were significantly higher than that in the Korean-Chinese nationality （ P 〈0.05）. CONCLUSION Choosing an auspicious day, benefit for fetus, intolerance to natural childbirth pain, safety and less pain of cesarean section and precious fetus were the main reasons of cesarean section without operation indications in puerperae in Yanbian area.
