慢性鼻窦炎合并骨炎的临床分析

摘　　要：目的 通过CT研究骨炎在慢性鼻窦炎中的发生率,探讨骨炎与慢性鼻窦炎病程及复发率之间的关系。方法对2010年1月~2014年5月期间本院收治的120例慢性鼻窦炎患者的手术治疗及临床资料进行回顾性分析,对资料进行统计学处理。结果 120例患者中39例发生骨炎,骨炎发生率32.50%（39/120）。GOSS量表评分以3分为多。病程在5年以内和5年以上的慢性鼻窦炎患者的骨炎发生率比较,差异有统计学意义（P〈0.05）。合并骨炎患者39例,不合并骨炎患者81例,两组患者术后复发率比较,差异有统计学意义（P〈0.05）。结论 慢性骨炎在慢性鼻窦炎中的发生率约为1/3,慢性鼻窦炎患者骨炎的发生率与病程呈明显正相关,合并骨炎的慢性鼻窦炎患者鼻内镜术后的复发率高于不合并骨炎者。手术彻底切除病变骨质,通畅引流是关键。Objective To discuss the relation between the course of disease and recurrence rate of osteitis and chronic sinusitis based on the incidence rate of osteitis in patients with chronic sinusitis as observed by CT. Methods The op- erative and clinical data of 120 patients with chronic sinusitis treated in our hospital from January 2010 to May 2014 were reviewed and statistically analyzed. Results 39 of the 120 patients were combined with osteitis, with an incidence rate of 32.50%（391120）, The scores of GOSS scale were mainly 3. There was significant difference in incidence of os- teitis between patients with a disease course of chronic sinusitis shorter than 5 years and those longer than 5 years （P〈 0.01）. There was significant difference in postoperative recurrence rate between the 39 patients combined with osteitis and the 81 patients without osteitis （P〈0.05）. Conclusion The incidence rate of chronic osteitis is about 1/3 among pa- tients with chronic sinusitis. There is a positive relationship between the incidence rate of 0steitis and the course of dis- ease. In addition, the recurrence rate after nasal endoscopy is higher in patients combined with osteitis than those with- out, Therefore, it is very important to thoroughly remove the affected bone substance and to keep adequate drainage.
