改良Stoppa入路与髂腹股沟入路手术治疗骨盆和髋臼骨折的临床效果比较

摘　　要：目的 对比分析改良Stoppa入路与髂腹股沟入路手术治疗骨盆和髋臼骨折的临床效果。方法 收集2013年1月~2015年5月期间来我院进行治疗的骨盆及髋臼骨折患者共73例作为研究对象,将73例患者按照治疗方法分组,研究组共36例采用改良Stoppa入路手术治疗,对照组共37例采用髂腹股沟入路手术治疗,比较两组手术切口长度、手术时间、术中出血量、骨折复位优良率、术后并发症情况。结果 与对照组比较,研究组手术切口长度更小,术中出血量更少,手术时间更短,骨折复位优良率更高,差异有统计学意义（P〈0.05）。研究组并发症发生率为5.56%,对照组为29.73%,两组术后并发症发生率比较差异有统计学意义（P〈0.05）。结论 骨盆及髋臼骨折患者用改良Stoppa入路手术治疗效果良好,优势显著,安全性高,建议在临床推广应用。Objective To compare and analyze the clinical effects of modified Stoppa approach and ilioinguinal ap- proach surgery in the treatment of pelvic and acetabular fractures. Methods 73 patients with pelvic and acetabular fractures who were admitted to our hospital for treatment from January 2013 to May 2015 were collected as the research subjects. 73 patients were assigned to groups according to the treatment approaches. 36 patients in the research group were given the surgery via the modified Stoppa approach, and 37 patients in the control group were given the surgery via ilioinguinal approach. Length of incision, surgery time, amount of bleeding during surgery, good rate of reduction of fractures and postoperative complications were compared between the two groups. Results The length of incision was smaller in the research group than that in the control group, amount of bleeding during surgery was fewer than that in the control group, surgery time was shorter than that in the control group, and the good rate of reduction of fractures was higher than that in the control group. The differences between the two groups were statistically significant （P〈0.05）. Incidence rate of complications was 5.56% in the research group and 29.73% in the control group, and the difference of incidence rate of complications after the surgery between the two groups was statistically significant （P〈0.05）. Conclu- sion The curative effects of modified Stoppa approach surgery for the patients with pelvic and acetabular fractures are favorable, with significant advantages and high safety, which is recommended to be promoted and applied in clinical settings.
