糖尿病对胰腺癌胰十二指肠切除术后并发症及生存情况的影响

摘　　要：目的探讨糖尿病对胰腺癌胰十二指肠切除术后并发症及生存情况的影响。方法选择2013年12月~2015年1月本院收治的胰腺癌患者80例,其中糖尿病组（40例）为合并2型糖尿病患者,无糖尿病组（40例）为明确排除合并糖尿病者,比较两组发生手术相关并发症情况、两组发生心脑血管相关并发症情况及两组中位生存时间及术后1个月生存质量。结果糖尿病组术后发生胰漏、胆漏、腹腔感染及肺部感染的总发生率显著高于无糖尿病组（P〈0.05）,糖尿病组发生低血压、高血压、心绞痛或心肌梗死及脑栓塞的比例显著高于无糖尿病组（P〈0.05）,糖尿病组中位生存时间短于无糖尿病组（P〈0.05）,生存质量低于无糖尿病组（P〈0.05）。结论糖尿病是导致胰腺癌胰十二指肠切除术后发生相关并发症的主要危险因素,且严重影响患者生存时间及生存质量。Objective To investigate the complications and survival of pancreatic pancreatoduodenectomy with Type 2diabetes.Methods 80 cases with pancreatic cancer from December 2013 to January 2015 in our hospital were choosed,the diabetes group（40 cases） with type 2 diabetes,without diabetes group（40 cases） for explicitly exclude diabetes mellitus.then suffered surgical complications related complications occurred two groups and cardiovascular the median survival time and quality of life after surgery in one month were compared.Results Type 2 diabetes group with postoperative pancreatic leak,bile leakage,intra-abdominal infections and lung infections overall incidence were significantly higher than that of non-diabetic group（P〈0.05）,type 2 diabetes group with hypotension,hypertension,angina pectoris or myocardial infarction and cerebral embolism proportion were significantly higher than that of non-diabetic group（P〈0.05）,diabetic group with the median survival time was shorter than that of non-diabetic group（P〈0.05）,life quality was worse than that of non-diabetes group（P〈0.05）.Conclusion Type 2 diabetes is the cause of pancreatic cancer pancreatoduodenectomy postoperative complications related to major risk factors and it is seriously affect the survival time and life quality.
