全髋关节置换术治疗成人髋关节发育不良继发骨关节炎的临床研究

摘　　要：目的探讨全髋关节置换术治疗成人髋关节发育不良继发骨性关节炎的疗效及髋关节发育不良的严重程度对手术疗效的影响。方法 39例43髋先天性髋关节发育不良继发骨性关节炎患者,Crowe分型Ⅰ型18例19髋,Ⅱ型15例16髋,Ⅲ型3例4髋,Ⅳ型3例4髋,行全髋关节置换术。观察疗效。结果本组患者患肢短缩明显改善,未发生假体松动、髋关节脱位等并发症。随访发现,平均Harris髋关节评分由术前(43.5±7.6)分,提高到术后(80.2±11.3)分(P<0.01)。结论全髋关节置换术是治疗成人髋关节先天性发育不良继发骨性关节炎的一种有效方法 ,而CroweⅠ型、Ⅱ型适应证较Ⅲ型、Ⅳ型更广。Objective To investigate curative effect by total hip arthroplasty in the treatment of secondary osteoarthritis in adult developmental dysplasia of the hip, and influence by developmental dysplasia of the hip on surgical effect. Methods A total of 39 cases （43 hips） patients with secondary osteoarthritis in congenital developmental dysplasia of the hip, as 18 Crowe Ⅰ type cases （19 hips）, 15 Ⅱ type cases （16 hips）, 3 Ⅲ type cases （4 hips） and 3 Ⅳ type cases （4 hips）, all received total hip arthroplasty. Their curative effects were observed. Results All patients had obvious improvement in short limb, without complications of prosthetic loosening and dislocation of hip joint. Follow-up showed increased mean Harris hip score from （43.5 ± 7.6） points before operation to （80.2 ± 11.3） points after operation （P〈0.01）. Conclusion Total hip arthroplasty is effective in treating secondary osteoarthritis in adult congenital developmental dysplasia of the hip. Crowe Ⅰ and Ⅱ types show more indications than Ⅲ and Ⅳ types.
