应用AO钛制弹性髓内钉治疗小儿股骨中段长斜形骨折的治疗体会

摘　　要：目的:探讨小儿股骨中段长斜形骨折手术治疗的一种方法。小儿股骨中段长斜形骨折是一种常见的损伤,约占全部小儿骨折的1.6%,由于小儿未发育成熟的骨骼有其自身特点,损伤后愈合特点也不能等同于成人,以往多采用牵引复位,石膏固定等保守治疗或切开复位钢板固定、外固定架固定等手术方法治疗。住院时间较长,护理困难,严重影响患儿的生活和学习。方法:对2012年1月-2013年7月间26例4~13岁小儿股骨中段长斜形骨折病人,用AO钛制弹性髓内钉内固定方法治疗;26例均为闭合性单侧肢体伤,男性16例,女性10例,左侧15例,右侧11例,均采用闭合复位逆行穿钉内固定。结果:术后随访4~12个月,平均7个月,26例患者骨折均愈合良好,无感染、畸形愈合、功能障碍等并发症,双下肢长短差平均不超过0.9cm。结论:适当合理地使用弹性髓内钉治疗儿童股骨中段长斜行骨折,安全、微创、愈合快,并发症少,是治疗小儿股骨中段长斜形骨折的一种有效方法。Objective： To explore one kind of surgical treatment for long oblique bone fracture in pediatric middle femoral shaft.Long oblique bone fracture in pediatric middle femoral shaft is a common injure,accounting for about 1.6% of all pediatric fractures.Due to the own characteristic of pediatric immature bone and its healing features after injuring different to those of adults,it usually adopts conservative treatments including traction reposition and plaster fixation,or surgical treatments such as open reduction treatment plate fixation and external fixation in fixed frame and etc.Long hospitalization time and difficult nursing significantly affects children patients＇ life and study.Method：From Jnanary 2012 to July 2013,there were 16 male and 10 fem^e cases,caused by trauma （15 cases was injuried in left side and 11 right）,4-13 years old and all patients were operated （close reduction and internal fixation with flexible nail from AO）.Result：26 patients were followed-up for 4 - 12 months, averge 7 months,and fusion was good without complication of delayed union,infection,malunion or functional disturbance.The length of double lower extremities was similar.Concluslon：Close reduction and internal fixation with flexible nail from AO is an effective way in the treatment of oblique fracture of femoral shaft in adolescent.
