基于齐多夫定和拉米夫定片方案的艾滋病抗病毒治疗的临床观察

摘　　要：目的评价艾滋病抗病毒治疗中齐多夫定和拉米夫定片方案(简称双夫定片方案)的疗效和安全性,为艾滋病防控和综合治疗提供科学依据。方法采集2014年1-12月在北京佑安医院接受基于双夫定片方案抗病毒治疗的艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人(简称HIV/AIDS病人)的临床资料,分析双夫定片方案的病毒学和免疫学疗效以及不良反应。结果在413例HIV/AIDS病人中,男男性行为感染占79.7%。初始治疗组113例,换药组300例(由其他药物或单成分药物转入双夫定片方案)。抗病毒治疗12个月时,初始治疗组病毒载量<50拷贝/mL者占92.0%。换药组病毒载量完全抑制者所占的比例由基线时的86.0%增加至抗病毒治疗12个月时的92.0%(P<0.05);CD4+T淋巴细胞计数由(315±177)个/μL升至(409±176)个/μL,差异有统计学意义(P<0.05)。不良反应以消化道症状和骨髓抑制为主,大多可以耐受。结论基于双夫定片的抗病毒治疗方案具有良好的病毒学和免疫学疗效,耐受性好。在发展中国家和资源有限地区的艾滋病抗病毒治疗中可能具有十分重要的作用。Objective To understand efficacy and safety of zidovudine-lamivudine tablets（AZT/3TC） in HIV an- tiretroviral therapy （ART） and provide a scientific basis for AIDS prevention and comprehensive treatment. Methods Clinical data were collected among HIV/AIDS patients treated with AZT/3TC-based regimens from January to December 2014 in our hospital, to analyze virological and immunological effects and adverse events of AZT/3TC in antiretroviral therapy. Results Of the 413 HIV/AIDS patients, 79.7 % were transmitted by homosexual behaviors. There were 113 patients in treatment-naive group, and 300 patients in drug switch group （other drugs or single-in- gredient drugs switched into AZT/3TC）. At 12 months of ART, the patients with HIV viral load less than 50 cop- ies/ml in treatment-naive group accounted for 92.0 %. Percentage of complete viral suppression in drug switch group significantly increased from 86.0 % at baseline to 92.0 % （P〈0.05）. CD4 cells increased from 315 4-177 cells/μL at baseline to 409± 176 cells/μL at 12 months of ART （P〈0.05）. Adverse reactions mainly consisted of gastrointesti- nal symptoms and bone marrow suppression, most of which were tolerable. Conclusion AZT/3TC-based antiretro- viral regimens have good virological and immunological efficacy, and are well tolerated. It may play a very important role in the ARV therapy in developing countries and resource-limited areas.
