不同病情程度糖尿病患者的PCT表达差异分析

摘　　要：目的探讨不同病情程度糖尿病(DM)患者的降钙素原(PCT)表达差异。方法以521例DM患者为研究对象,采用APACHEII评分、尿微量白蛋白(Um Alb)等指标评估患者的病情严重程度,检测其血PCT水平,分析不同病情严重程度DM组间PCT值、IL-6、TNF-α、INF-γ的差异,并进一步分析病情严重程度与PCT水平的相关性。结果随DM病情程度的加重,DM患者PCT值、IL-6值和TNF-α值呈升高趋势(P<0.05),而INF-γ值呈降低趋势(P<0.05)。在合并慢性并发症组中,PCT的表达水平与Um Alb水平呈正相关(r=0.705,P<0.01);而在合并急性并发症组中,PCT的表达水平与APACHEII评分呈正相关(r=0.874,P<0.01)。结论在DM患者,PCT表达水平与患者的病情程度密切相关,可作为判断患者病情严重程度的指标之一,其可能对DM病情诊断具有重要的鉴别价值。Objective To explore the expression difference of procalcitonin （PCT） among patients with diabetes mellitus （DM） of different severity. Methods A total of 521 DM patients were selected to evaluate the severity by APACHEII scores, UmAlb and other indicators, detect the PCT levels, analyze the difference of PCT, IL-6, TNF-a and INF-γ, among groups at different DM severity, and further analyze the correlation between the severity and PCT levels. Results The PCT, IL-6 and TNF-α in DM patients increased with the deterioration of DM severity （P 〈 0.05）, but INF-γ, dropped （P 〈 0.05）. In the group with chronic complications, the levels of PCT were positively related to UmAlb levels （r = 0.705; P 〈 0.01）, while in the group with acute complications, the levels of PCT were positively related to APACHEII scores （r = 0.874; P 〈 0.01）. Conclusion For DM patients, the levels of PCT are closely related to the severity, thus it can be used as one of the indicators for judging the severity of DM and may have significant value in differential diagnosis of DM severity.
