摘  要:目的探讨烧伤患者体内磷酸化胰岛素样生长因子结合蛋白1(insulin-like growth factor binding protein-1,IGFBP-1)含量水平变化。方法选取2012年12月-2014年12月收治的86例烧伤患者为研究对象,收集烧伤患者自入院治疗至术后治疗10d的血浆,酶联免疫吸附法(ELISA)测定血浆中胰岛素、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)和IGFBP-1含量,磷酸化IGFBP-1用Western Blotting检测。结果烧伤患者血浆IGFBP-1含量与总体表面积(TBSA)烧伤指数呈正相关,IGF-1含量随着TBSA值升高而显著下降(P〈0.05)。在患者烧伤后72h内,IGF-1含量降低,而IGFBP-1含量[(184±26)ng/m L]升高,相比正常健康组IGFBP-1含量[(16±4)ng/m L]结果有显著统计学意义(P〈0.05),同时血浆中可检测到磷酸化的IGFBP-1。结论烧伤患者血浆IGFBP-1含量变化和IGFBP-1持续的磷酸化与烧伤严重程度相关,IGFBP-1持续的磷酸化可抑制IGF的生物活性,并可作为判断烧伤患者预后的预警指标。Objective To investigate the changes of insulin-like growth factor binding protein 1( IGFBP-1)in patients with burn injury. Methods Eighty-six adult patients with burn injury who were admitted in our hospital from Dec. 2012 to Dec. 2014 were selected as study subjects. Plasma was collected from the date of admission to 10 days after admission. The plasma levels of insulin,insulin-like growth factor-1( IGF-1) and IGFBP-1 were measured by ELISA. Phosphorylated IGFBP-1 was detected by Western Blotting. Results The plasma level of IGFBP-1 was positively correlated with the total body surface area( TBSA). The plasma level of IGF-1 was decreased significantly with the increased degree of TBSA in burn patients. The plasma level of IGF-1 decreased within 72 h after burn injury. However,the mean IGFBP-1 level [( 184 ± 26) ng / m L] was significantly higher than that of normal health subjects [( 16 ± 4) ng / m L,P〈0. 05]. The phosphorylated IGFBP-1 could be detected in plasma in patients with burn injury. Conclusion The changes of IGFBP-1 and persistent phosphorylation of IGFBP-1 in plasma of patients with burn injury are positively correlated with the severity of burn injury. The sustained phosphorylation of IGFBP-1 can inhibit the biological activity of IGF,and can be accepted as a predictor in predicting the prognosis of burn injury.
