扩张型心肌病患者血清半乳糖凝集素-3水平变化及其与胶原代谢标志物的相关性分析

摘　　要：目的 分析扩张型心肌病患者血清半乳糖凝集素-3（Galectin-3）水平的变化及其与Ⅰ型前胶原羧基端肽（PIP）、Ⅰ型胶原羧端交联肽（CITP）的相关性。方法 纳入2014年6月—2015年12月苏州市立医院心内科收治的扩张型心肌病患者（NYHAⅢ~Ⅳ级）31例作为观察组,同期选取20例心功能正常的体检者作为健康对照组。分析比较2组血清Galectin-3、PIP、CITP浓度及其相关性。结果 观察组Galectin-3水平明显高于健康对照组[15.85（11.7,22.0）ng/rnl vs.12.79（8.23,16.36）ng/ml,P=0.038],PIP水平高于健康对照组[（65.34±21.60）μg/L vs.（32.78±14.45）μg/L,t=5.93,P=0.001],CITP水平高于健康对照组[（6.35±2.35）μg/L vs.（5.14±2.32）μg/L],但差异无统计学意义（t=1.80,P=0.083）。相关分析提示Galectin-3与PIP呈正相关关系（r=0.325,P=0.035）,Galectin-3与CITP之间无明确相关关系（r=0.087,P=0.560）。结论 扩张型心肌病患者Galectin-3、PIP水平明显高于健康人群,并且两者存在正相关关系。提示Galectin-3可能通过干扰Ⅰ型胶原代谢过程而参与扩张型心肌病的心肌纤维化以及心肌重构过程。Objective To identify the associates of Galectin 3 in dilated cardiomyopathy（DCM） patients ,and clarify the relationship with PIP（carboxyl terminal peptide of procollagen type I ） and CITP（carboxyl terminal telopeptide of collagen type I）, biomarkers of collagen type Ⅰmetabolism.Methods From June 2014 to December 2015, in Suzhou Municipal Hospital, Department of Cardiology, patients were dilated cardiomyopathy （NYHA class III -IV） of 31 cases as the observa-tion group were enrolled , and selected at the same period 20 cases of normal cardiac function examination as healthy control group.The concentrations of serum Galectin 3, PIP and CITP in 2 groups were analyzed and compared .Results Observation group＇ s galectin 3 levels was significantly higher than those in healthy control group [15.85 （11.7, 22.0） ng/ml vs.12.79 （8.23, 16.36） ng/ml, P =0.038], PIP level was higher than that of the healthy control group [（65.34 ±21.60） μg/L vs. （32.78 ±14.45）μg/L, t =5.93, P =0.001], CITP level is higher than that of the healthy control group [（6.35 ±2.35）μg/L vs.（5.14 ±2.32）μg/L], but the difference was not statistically significant （ t =1.80, P =0.083）.Correlation a-nalysis indicated that there was a positive correlation between PIP and Galectin 3 （ r =0.325, P =0.035）, and there was no clear correlation between CITP and Galectin 3 （ r =0.087, P =0.560）.Conclusion The level of Galectin 3 and PIP in pa-tients with dilated cardiomyopathy was significantly higher than that in healthy people , and there was a positive correlation be-tween them.It suggested that Galectin 3 might be involved in the process of myocardial fibrosis and remodeling of dilated car-diomyopathy by interfering with the metabolism of type I collagen .
