摘 要:目的 观察 ω-3多不饱和脂肪酸(PUFA)通过抗炎抗氧化作用对出血性休克复苏期损伤的保护作用.方法 取雄性SD大鼠80只,均分为对照组,模型组以及ω-3PUFA低(10 mg/kg)、中(30 mg/kg)、高剂量组(100 mg/kg),除对照组外其他各组大鼠建立出血性休克模型,待进入复苏期后记录各组大鼠的心率、平均动脉血压和血氧饱和度,并检测致炎细胞因子肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β和内毒素水平,以及氧化应激相关的丙二醛(MDA)、超氧化物歧化酶(SOD)和髓过氧化物酶体(MPO)等氧化应激指标的变化.结果 在复苏期30 min ω-3PUFA高剂量组大鼠的平均动脉压、心率和动脉血氧分压分别为105.1 mmHg(1 mmHg =0.133 kPa)、319.2次/分和69.4 mmHg,显著高于模型组的69.6 mmHg、310.5次/分和38.1 mmHg;ω-3PUFA可以升高大鼠在失血性休克后复苏期的平均动脉压和动脉血氧分压指标;同时,ω-3PUFA还可以显著降低失血性休克复苏期TNF-α 、IL-1β、MDA、MPO和内毒素的含量,增加SOD的含量.结论 ω-3PUFA对大鼠失血性休克复苏期具有良好的保护作用,其机制与其抗炎及抗氧化应激有关。Objective This study tests the hypothesis that treatment with ω-3 polyunsaturated fatty acid (ω-3PUFA) will attenuate inflammatory responses and stabilize hemodynamics.Methods Male SD rats were randomly divided into 5 groups:control,model with hemorrhagic shock/resuscitation (HS/R) and ω-3PUFA low,medium and high dose group,each agent was administered before resuscitation.The model was performed and fish oil (10,30,100 mg/kg) was infused through caudal vena at 30 minutes after shock.Half of each group was killed at 30 minutes and at 2 hours after resuscitation.Then several kinds of inflammation and oxidative stress indicators such as interleukin (IL)-113,myeloperoxidase (MPO),tumor necrosis factor-α (TNF-o),endotoxin and superoxide dismutase (SOD) were tested.Results The mean anerial pressure (MAP) and arterial partial pressure of oxygen (PaO2) of high dose ω-3PUFA group was 105.1 mmHg (1 mmHg =O.133 kPa) and 69.4 mmHg,significantly higher than model group (69.6 mmHg,38.1 mmHg,P 〈 0.05,respectively);ω-3PUFA could increase the MAP and PaO2 in hemorrhagic shock in resuscitation period.In addition,ω-3PUFA could significantly reduce the levels of TNF-α,IL-1β,malondialdehyde (MDA),MPO and endotoxin in serum and increase SOD level.Conclusion ω-3PUFA pretreatment before fluid resuscitation showed a beneficial effect to the hemodynamic stabilization through anti-inflammation and anti-oxidase in HS/R rat model.
