摘  要:目的:分析肥胖儿童血清脂肪细胞因子水平与糖脂代谢、微炎症状态的关系。方法:299例肥胖患儿及264例正常儿童纳入本次研究,抽取空腹外周静脉血测定血清脂肪细胞因子、糖脂代谢及微炎症相关指标水平,进一步分析肥胖儿童的脂肪细胞因子水平与糖脂代谢、微炎症相关指标水平的相关关系。结果:观察组血清瘦素(leptin)、Vaspin水平高于对照组儿童,脂联素(APN)水平低于对照组(P<0.05);血清空腹胰岛素(FINS)、C-P、皮质醇(Cor)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平高于对照组,高密度脂蛋白胆固醇(HDL-C)水平低于对照组(P<0.05);血清超敏C反应蛋白(hs-CRP)、白介素-8(IL-8)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平高于对照组(P<0.05);血清leptin、APN、Vaspin水平与以上糖脂代谢及微炎症状态指标水平均存在直接相关关系。结论:肥胖儿童存在炎症因子高表达、糖脂代谢紊乱,过度表达的脂肪细胞因子可能是肥胖持续存在并加重的重要因素。Objective: To analyze the relationship of serum adipocytokine levels with glycolipid metabolism and micro- inflammatory state in obese children. Methods: A total oF 299 cases of obese children and 264 cases of normal children were in- cluded in the study, fasting peripheral venous blood was extracted to determine serum levels of adipocytokines, glucolipid me- tabolism and micro-inflammation-related indexes, and the correlation between the levels of adipocytokines and the levels of glu- colipid metabolism and micro-inflammation-related indexes was further analyzed. Results: Serum Leptin and Vaspin levels of observation group were significantly higher than those of control group, and APN level was significantly lower than that of con- trol group (P〈0.05) ; serum FINS, C-P, Cor, TG and LDL-C levels were significantly higher than those of control group,and HDL-C level was significantly lower than that of control group (P〈0.05); serum hs-CRP, IL-8, IL-6 and TNF-α levels were significantly higher than those of control group (P〈0.05) ; serum Leptin, APN and Vaspin levels were directly correlated with the levels of above glucolipid metabolism and micro-micro-inflammatory state indexes. Conclusions.. There are high ex- pression levels of inflammatory factors and glucolipid metabolism disorder in obese children, and excessively expressed adipocy- tokines may be the important factors of persist and worsened obesity.
