基于Greene量表评价疏肝调经针灸方案治疗围绝经期情绪障碍的疗效

摘　　要：目的:基于Greene量表探讨疏肝调经针灸方案治疗围绝经期情绪障碍的疗效。方法:经筛选后具有抑郁或焦虑的围绝经期女性患者80例随机等分为A-D 4组,分别对应采用盐酸帕罗西汀、利维爱、帕罗西汀+利维爱和疏肝调经针灸方案治疗8周。所有患者在治疗前后各进行1次Greene及其因子评定,并在观察期结束时进行TESS评定。结果:Greene总分差及焦虑因子分差:4种治疗方法均能改善,且相互间差异无统计学意义;抑郁因子分差:与A组比较,B组不能有效改善(P<0.01),C组和D组优于A组及B组(P<0.01),且C组和D组间无统计学意义;性因子和血管因子分差:与A组比较,其余3组均优于A组(P<0.01),且后3组间差异无统计学意义;TESS评分:A、B、C组均高于D组(P<0.01)。结论:对绝经期情绪障碍的治疗中,考虑到盐酸帕罗西汀和利维爱的不良反应,程氏疏肝调经针灸方案值得推荐。Objective：To explore the curative effect of treating women emotional disorder during perimenopause with acupuncture plan of dispersing liver and regulating menstruation based on Greene scale.Methods：Eight female patients with depression and anxiety during perimenopause were selected and randomly divided into 4 groups（group A to group D）,and patients in the 4 groups were treated with paroxetine,Livial,paroxetine＋Livial,and acupuncture plan of dispersing liver and regulating menstruation respectively for 8 weeks.The Greene and factor in all the patients before and after treatment were evaluated,and TESS in all the patients was evaluated at the end of the observation period.Results：The 4 treatments all can improve the Greene score and anxiety factor score,and the difference between the 4 treatments was not significant.Compared with group A,the depression factor score of patients in group B was not improved effectively（P〈0.01）,the depression factor score of patients in group C and D was superior to that of group A and B（P〈0.01）,and the difference between group C and D was not significant.The sex factor score and vascular factor score of patients in group B,C and D were superior to that of group A significantly（P〈0.01）,and the difference between group B,C and D was not significant.The difference in TESS score between the i rst three groups and group D was significant（P〈0.01）.Conclusion：In the treatment of this disease,considering the side effects of paroxetine and potential long-term risk of Livial,Chengs＇ acupuncture plan of dispersing liver and regulating menstruation is worth recommend.
