肾康注射液联合吗替麦考酚酯治疗难治性肾病综合征的临床疗效观察

摘　　要：目的:观察肾康注射液联合吗替麦考酚脂(MMF)治疗难治性肾病综合征(refractory nephrotic syndrome,RNS)的临床效果。方法:按照随机对照原则,将72例RNS患者随机分为治疗组36例(基础治疗+肾康注射液+吗替麦考酚脂)和对照组36例(基础治疗+吗替麦考酚脂)。治疗前及治疗后分别检测24h尿蛋白定量、肾功能、血白蛋白、血脂、肝功能、血常规、凝血功能等,并观察两组不良反应。结果:治疗后治疗组尿蛋白明显减少,血浆白蛋白升高,凝血功能改善,血脂逐渐下降,均有显著性差异(P<0.05)。对照组在尿蛋白、血浆白蛋白、血脂方面与治疗前比较,有显著性差异(P<0.05)。治疗组显效22例,有效10例,无效3例;对照组显效8例,有效13例,无效15例;两组间疗效比较有显著差异(P<0.05)。结论:肾康注射液联合吗替麦考酚脂治疗RNS有疗效显著,副作用少。Objective： To observe the clinical curative effect of Shenkang Injection combined with Mycophe- nolate mofetil on Refractory nephrotic syndrome. Methods： according to the randomized and controlled princi- ples, 72 patients with RNS were randomly divided into a treatment group and a control group. The treatment group had 36 patients treated with basic therapy, Shenkang Injection and MMF, while the control one had 36 patients treated with basic therapy and MMF. Twenty-four hour urinary protein quantitate, renal function, serum albumin, blood fat, hepatic function, blood routine were detected respectively and compared before and after the treatment, then the adverse reaction and side effect of the two groups were observed. Methods： Urine pro- tein was obviously decreased, plasma albumin was gradually increased, and blood fat was gradually lowered in the treatment group after treatment. There were significant differences respectively （P 〈 0. 05 ）. Compared with the treatment group, urine protein, plasma albumin and blood fat in the control group had significant differences （P 〈0. 05）. In the treatment group, 22 cases had excellent effect, 10 cases were effective, and 3 cases were ineffective; In the control group, 8 cases had excellent effect, 13 cases were effective, and 15 cases were inef- fective. To compare the effects between the two groups, they had significant differences （ Z = - 2. 6355, P = 0. 026）. Conclusion： It has a better effect when Shenkang Injection is used with MMF on RNS, and it can reduce side effects. Refractory nephrofic syndrome.
