摘 要:目的探讨尿液中性粒细胞明胶酶相关脂质运载蛋白(NGAL)检测在急性肾功能损伤(AKI)早期诊断中的意义。方法收集肾移植术后并发AKI患者36例,根据其病情严重程度将AKI患者分为3个亚组:危险(R)组、损伤(I)组和衰竭组(R)组,同时随机选取同期健康体检者29例(NC)作为对照,分析不同级别的患者尿液NGAL水平,结合传统指标、临床表现,综合评估NGAL对诊断AKI的临床应用价值。结果NC组和AKI组两组在SBUN、SCr、e GFR及UNGAL水平进行方差分析,两组SBUN水平差异无统计学意义(P>0.05),在SCr、e GFR及UNGAL水平,两组差异有统计学意义(P<0.05),各亚组在SCr、e GFR及UNGAL水平,差异均有统计学意义(P<0.05),经Pearson检验及线性回归分析,UNGAL水平与e GFR水平呈负相关,r值为-0.632(P<0.01)。结论尿液NGAL的水平的检测过程简便易行,有助于肾移植术后并急性肾损伤的早期诊断和预后评估,可作为肾移植术后高危人群的常规检查在临床工作中推广应用。Objective To explore the significance of urine neutrophil gelatinase associated lipocalin (NGAL) detection in the early diagnosis of acute renal injury (AKI). Methods 36 cases of patients with AKI after renal transplantation were collected. According to the severity of the disease, they were divided into hazard (R) group, injury (I) group and failure group. At the same time, 29 cases of physical health person over the same period were randomly selected as control (NC). Levels of NGAL in urine of patients at different levels were analyzed. Combined with the traditional indicators, clinical manifestations, the clinical application value of NGAL in the diagnosis of AKI was comprehenSively evaluated. Results SBUN, SCr, eGFR and UNGAL levels of NC group and AKI group were analyzed by analysis of variance. The level of SBUN in the two groups was not statistically significant (P 〉 0.05). There were statistical significances in SCr, eGFR and UNGAL levels of the two groups (P 〈 0.05). There were statistical significances in SCr, eGFR and UNGAL levels of every subgroup (P 〈 0.05). By pearson test and linear regression analysis, the level of UNGAL was negatively correlated with the level of Egfr, and R value was -0.632 (P 〈 0.01). Conclusion Detection of urine NGAL levels is simple and easy to operate. It is conducive for early diagnosis and prognosis of acute renal injury after renal transplantation. It can be used as a routine examination in patients at high risk after renal transplantation to popularize and apply in clinical work.
