摘  要:目的探讨早期心肌缺血猝死患者心肌干细胞及高迁移率族蛋白1(HMGB1)、mi RNA-206表达的变化。方法收集中国人民解放军第三军医大学第一附属医院2012年10月至2015年10月的100例尸检标本,早期心肌缺血猝死组(n=40),心肌梗死组(n=30),正常心肌组(n=30),运用免疫组织化学二步法染色,采用卡方检验比较分析早期心肌缺血组、心肌梗死猝死组、正常心肌组中心肌干细胞、HMGB1及mi RNA-206的表达情况,并采用Spearman相关系数法分析早期心肌缺血组中HMGB1与mi RNA-206表达的相关性。结果早期心肌缺血猝死组中HMGB1的阳性表达率为97.50﹪(39/40)与心肌梗死猝死组100﹪(30/30)比较差异无统计学意义(c^2=0.761,P=0.383),但二者均显著高于正常心肌组的3.33﹪(1/30),差异具有统计学意义(c^2=273.297,P=0.000);早期心肌缺血猝死组中c-kit+的阳性表达率为77.50﹪(31/40)与心肌梗死猝死组86.67﹪(26/30)比较差异无统计学意义(c^2=0.953,P=0.329),但二者明显高于正常心肌组的10.00﹪(3/30),差异有统计学意义(c^2=2163.431,P=0.003);早期心肌缺血猝死组中mi RNA-206的表达为正常心肌组的3.02±0.53倍,心肌梗死猝死组中的表达是正常心肌组的4.63±0.71倍,差异有统计学意义(t=10.870,P=0.000);早期心肌缺血猝死组中HMGB1的阳性表达率为97.50﹪(39/40)明显高于c-kit+阳性表达率的77.50﹪(31/40),两组比较差异有统计学意义(c2=0.824,P=0.031)。结论心肌干细胞及HMGB1、mi RNA-206在早期心肌缺血猝死中阳性表达率比较高,正常心肌中均为弱性表达,比较分析三者的差异性表达,可以为早期心肌缺血猝死的死因鉴定提供有效的客观指标。ObjectiveTo investigate the variety of cardiac stem cells and high mobility group box 1 protein (HMGB1), miRNA-206 expression in patients with early acute myocardial ischemic. MethodsA total number of 100 autopsy specimens from October 2012 to October 2015 were collected and divided into early myocardial ischemia sudden death group(n = 40), myocardial infarction group(n = 30), and normal myocardium group(n = 30). Two-step immunohistochemical staining,Chi-square test to compare analyzes the expression of early myocardial ischemia group, myocardial infarction, sudden death, and normal cardiac muscle stem cells in the center of the group, HMGB1 and miRNA-206, and the analysis of early myocardial ischemia group HMGB1 and miRNA- using Spearman correlation coefficient correlation between the expression of 206. ResultsThe positive expression rate of HMGB1was 97.50﹪(39/40)and 100﹪(30/30)in early myocardial ischemia sudden death group and myocardial infarction group respectively with no significant difference(c2= 0.761, P = 0.383), but both were significantly higher than 3.33﹪(1/30)in normal myocardium group, which was statistically significant(c2= 273.297, P = 0.000); The expression of early myocardial ischemia and sudden death group, the rate of c-kit+ 77.50﹪(31/40)and myocardial infarction, sudden death group 86.67﹪(26/30)the difference was not statistically significant(c2 = 0.953, P = 0.329), yet both of which were markedly higher than that in normal cardiac group(10.00﹪(3/30), c2=2163.431,P = 0.003). miRNA-206 expression in early myocardial ischemia sudden death group and myocardial infarction group was 3.02 ± 0.53 and 4.63 ± 0.71 times as much as in normal cardiac group respectively. The difference was statistically significant(t = 10.8698,P = 0.000).
