胸椎后纵韧带骨化合并黄韧带骨化采用后路全椎板切除减压并椎弓根内固定治疗的分析

摘　　要：目的探究在胸椎后纵韧带骨化合并黄韧带骨化的临床治疗中后路全椎板切除减压联合椎弓根内固定治疗的应用效果。方法：收治胸椎后纵韧带骨化合并黄韧带骨化患者77例，患者采用后路全椎板切除减压和椎弓根内固定治疗。结果；患者在手术后随访12—28个月，平均为20．72个月，患者治疗后，其JOA评分有显著的改善（P〈0．05），其中2例患者在手术过程中出现脑脊液漏病症，3例出现伤口感染，2例出现血肿病症。结论：在胸椎后纵韧带骨化合并黄韧带骨化的临床治疗中结合后路全椎板切除减压和椎弓根内固定治疗能够显著改善患者的相关指征，同时降低术后并发症，值得在骨科治疗中推广应用。Objective To explore the longitudinal ligament after thoracic ossification merge all yellow ligament ossification in the clinical treatment of posterior vertebral lamina resection decompression joint pedicle internal fixation treatment of application effect. Methods： 2012 moon people during December, 2013, our hospital for treatment of thoracic longitudinal ligament ossification after merger yellow ligament ossification, 77 cases of patients, patients with posterior vertebral lamina resection decompression and pedicle internal fixation treatment, the clinical treatment of patients with retrospective statistical analysis of relevant data. Results： The patients were followed up for 19-28 months after surgery, with an average of 20.72 months, patients in the treatment of three months and the last follow-up JOA score of 3.87 points, 8.15 points and 8.15 points respectively, its score significantly improved after treatment （P〈0.05）, among them 2 patients cerebrospinal fluid leakage in the process of operation condition, 3 patients after surgery of wound infection, 2 cases of patients with hcmatoma. Conclusion： Longitudinal ligament ossification merge after thoracic yellow ligament ossification in the clinical treatment combined with posterior vertebral lamina resection decompression and pedicle internal fixation treatment can significantly improve the patient＇s relevant indications, at the same time reduce the postoperative complications, worthy of popularization and application in the orthopaedic treatment.
