117例新生儿医院感染败血症病原菌及耐药性分析

摘  要:目的调查分析新生儿医院感染败血症的病原菌分布、药敏情况。方法:回顾性分析我院新生儿科2012年5月-2015年4月发生医院感染败血症的新生儿临床资料,分析病原菌的分布及耐药性,比较其临床特点。结果:研究期间收治患儿7159例,医院感染败血症发生率1.63%(117/7159)。其中,足月儿10例,早产儿107例,深静脉置管64例。早产儿医院感染败血症占同期住院新生儿的1.49%(107/7159)。病原菌以革兰阴性菌最多见(69.23%),革兰阳性菌占第二位(17.09%),真菌占第三位(13.68%)。其中,革兰阴性杆菌以肺炎克雷伯菌为主,对大部分且内酰胺类抗生素耐药:革兰阳性菌以表皮葡萄球菌为主,大多对青霉素耐药,对万古霉素敏感:真菌感染均为念珠菌,对氟康唑均敏感。结论:新生儿医院感染败血症以早产儿为主,检出病原菌前3位是肺炎克雷伯菌、真菌及大肠埃希菌,细菌耐药性严重,临床应合理应用抗菌药物,减少耐药株及真菌的出现。Objective To evaluate the pathogen distribution and the clinical characteristics in newborn infants with nosocomial sepsis. Methods:A retrospective chart review was conducted.The data of all hospitalized preterm infants with nosecomial sepsis from May 2012 to April 2015 were collected and analyzed. Results:In the study cohort,nosocomial sepsis occurred in 117 newborn infants.The incidence was 1.63%. Among those 117 eases,92.31% newborn infants with nosocomlal sepsis were cured.The leading pathogens were gram negative bacteria and gram-positive organisms.The main pathogen of gram-negative organisms was Klebsiella and the main pathogen of gram-positive organisms was Staphylococcus epidermidis.Candida was responsible for all fungal sepsis.Most gram-negative organisms were resistant to common-lactam antibiotics.Most gram-positive organisms were resistant to penicillin and sensitive to vancomycin.All of the fungi were sensitive to fluconazole.Conclusions:Most of the nosocomlal infection in neonatal septicemia in premature infants,detection of pathogenic bacteria of three is Klebsiella pneumoniae bacteria,fungi and Escheriehia coli,bacterial drug resistance is serious, clinical should rational use of antimicrubial drugs,reduce the emergence of resistant strains and fungal.
