成年男性腹股沟后壁的测量与疝补片尺寸选择以及Rutkow术后效果分析

摘　　要：目的测量中国籍成年男性人群中的腹股沟后壁大小，藉以提供更适合中国成年男性腹股沟解剖结构的补片尺寸数据，有助于国人选择大小合适的疝补片，以及分析Rutkow术后效果。方法：对行无张力疝修补术180例中国籍成年男性患者的腹股沟后壁大小的精细测量，计算出补片尺寸数据，并将180例患者随机分为两组：每组90例，实验组行Rutkow术，对照组行Lichtenstein术，比较两组患者的一般情况、术后并发症以及复发率。结果：耻骨结节与腹内斜肌在腹股沟韧带起点之间距离均数为（6．03±0．58）cm；弓状下缘的最高点至腹股沟韧带的垂直距离均数为（3．02±0．32）cm。实验组下床活动时间、进食时间、住院时间均小于对照组（P〈0．05）；两组手术时间、恢复正常活动时间比较，差异无统计学意义（P〉O．05）：实验组术后并发症发生率及复发率，明显低于对照组（P〈0．01）。结论：根据本研究结果，相应补片尺寸长度应当大于（13．04±0．74）Cm，相应补片尺寸宽度应当大于（5．88±0．52）cm。Rutkow术用于腹股沟疝修补，效果明显，并发症和复发率低，值得临床推广应用。Objective Measurement of Chinese nationality adult men in posterior wall of inguinal hernia size, in order to provide more suitable for Chinese adultmale inguinal tissue anatomical structure of patch size data, help to our choice of suitable size of hernia patch,and analysis of the postoperative eftect of Rutkowprocedure. Methods： A fine measure of the size of the posterior wall of 180 Chinese adult male patients who underwent mesh-plug tension-free heruioplasty inRenmin Hospital of Wuhan University from January 2014 to November 2015 were calculated. And 180 cases were randomly divided into two groups：90 cases ineach group, the experimental group was performed Rutkow, with reference to the group Liehtenstein, compared with the general situation of the two groups,postoperative complications and recurrence rate. Results： The pubic tubercle to internal oblique inguinal ligament between the starting point in the distance meanto 6.03 ± 0.58cm ; the vertical distance between the highest point on the edge of the arcuate to inguinal ligament mean to 3.02 ± 0.32cm. The time for dietingand hospitalization in the experimental group was significantly less than the reference group （P〈0.05）, the time of operation and recover＂ of normal activitiesshowed no statistically significant difference between the two groups （P〉0.05）; the rate of postoperative complication and recurrence in the experimental group wassignificantly lower than the reference group （P〈0.01）.Conclusions：Aceording to the data of our measurements, the size of the corresponding patch should begreater than 13.04±0.74cm, and the corresponding patch size should be greater than 5.88±0.52cm. The benefit of Rutkow procedure used for inguinal herniarepair is obvious, the complication and recurrence rate is low, it is worthy of clinical application.
