复杂胫骨远端骨折的治疗体会

摘　　要：目的探讨复杂胫骨远端骨折的治疗技巧。方法：2010年1月-2015年6月共收治胫骨远端粉碎性骨折48例，AO分型全部为c型，治疗均采用锁定钢板或者锁定钢板与外支架、螺钉相结合的方案。结果：48例患者，随访时间12—24个月，平均随访18个月，全部患者骨折均愈合，愈合时间为3-9个月，平均6．1个月，疗效按照Johner—Wruhs评分标准评定，优28例，良14例，可6例，优良率87．5％，2例切口局部皮肤坏死，一例换药愈合，一例局部皮瓣修复。结论：胫骨远端粉碎性骨折经过合理科学的手术治疗依然能够获得良好的功能恢复。ObjectiveTo explore the treatment techniques on cormninuted fracture of distal tibia. Methods： From Jan. 2010 to Jun. 2015, 48 patients with distaltibia comminuted fracture in our department,according to the AO fracture classification, all the patients were C-type fractures, and were treated by lucking plateor locking plate combined with external fixator and screws. Results：All 48 patients were followed from 12-14 nmnths to 24 months with tile average of 18 months,all the fractures were all healed, the healing time from 3-9months with an average healing time of 6.1 months, Johner-Wruhs method was used to evaluate thetreatment effect, 28 cases were excellent, 14 cases were good and 6 cases were better, the excellent and good rate was 87.5%. 2 case.＇.; with local skin necrosisaround incision, 1 case was healed by dressing change and I case was healed by local flaps reconstruction. Conclusions：Patients with comminuted fracture of dis-tat tibia can still get good functional recovery by reasonable surgical treatment.
