0°视角内镜下第三脑室底造瘘治疗非交通性脑积水的临床转归观察

摘　　要：目的探讨0°视角神经内镜下第三脑室底造瘘治疗非交通性脑积水的临床转归。方法：纳入31例非交通性脑积水患者，均采用0°视角内镜下行脑室镜下行第三脑室底造瘘治疗。结果：29例患者术后短期疗效满意。随访中，1例症状复发改行脑室一腹腔分流术，无严重并发症。术后6个月VCR的减小以及mRS预后与术前比较，差异存在统计学意义（P〈0．05）。结论：通过0°视角内镜进行第三脑室底造瘘术可迅速有效地改善非交通性脑积水症状，脑脊液通过瘘孔进入生理性循环，可避免感染和过度分流，操作简单，效果安全可靠。Objective To investigate the clinical outcome of 0 degree angle of view on the treatment of non communicating hydrocephaluswith the first three ventricle floor of the nerve endoscope. Methods：On the standard of 31 cases of non traffic cerebral hydrocephalus patientswere treated with 0 degree view endoscope ventriculoscope down the floor of the third ventricle made fistula treatment. Results：The short-termcurative effect was satisfactory in 29 cases. During follow-up, 1 cases of recurrence of symptoms via ventriculoperitoneal shunt, no seriouscomplications. There was statistical significance （P〈0．05） in the reduction of VCR in June and the difference in the prognosis of mRS.Conclusion：By endoscopy view angle of 0 DEG to the floor of the third ventricle made fistula can quickly and effectively improve the noncommunicating hydrocephalus symptoms, cerebrospinal fluid through the fistula hole into the physiological cycle, can prevent infection andexcessive diversion, simple operation, effect is safe and reliable.
