伴结肠痉挛STC患者肠壁组织PGP9.5的表达及意义

摘  要:目的探讨慢传输型便秘患者结肠痉挛与肠神经系统之间的关系。以期更深入地揭示STC患者伴发结肠痉挛症状的病理生理基础。方法:选取外科手术治疗的STC患者。影像学检查和(或)手术当中发现有肠管痉挛的STC患者为实验组,共30例;影像学检查及手术时均未发现肠道痉挛的患者为对照组,共30例。应用免疫组化方法对肠壁神经系统中的PGP9.5进行检测,观察神经系统中神经元的数量,并观察其形态变化。结果:与对照组比较,实验组痉挛段结肠粘膜下层和肌层神经节细胞数均较少(P〈0.05)。实验组和对照组横结肠粘膜下层、肌层节细胞数差异不明显,无明显统计学差异(P均〉0.05)。光镜下两组肠壁粘膜下层、肌层均可见黄褐色团块状的PGP9.5阳性反应的神经节细胞。伴有结肠痉挛的实验组肠壁内PGP9.5阳性反应的神经节细胞体积较大,但数量较少,染色相对较浅。结论:STC患者局部结肠痉挛与肠神经系统神经节细胞的数量进一步减少以及发育不良有关。Objective To explore the relationship between colonic spasm and enteric nervous system in patients with slow transit constipation,in order to reveal the pathophysiological basis of STC in patients with cotonospasm.Methods:STC patients with surgical treatment were selected.30 STC patients with colonospasm found by imaging of X-ray and (or) surgery were selected as experimental group.30 STC patients withoutcolonospasm were selected as control group. The expressions of PGP9.5 in enteric nervous system were detected by immunohistoehemistrymethod. Then the number of the ganglion cells and morphological changes in the nervous system were observed. Results:The number ofganglion cells in spastic colon submucosal and muscular layer in experimental group was less than that in control group (P〈0.05). There wereno significant differences in number of ganglion cells of transverse colon submucosal and muscle layer between both groups (all P〉0.05).Theganglion cells of PGPg.5 positive reaction under two groups of colon submucosa and muscular layer were visible brown lumpish under lightmicroscopy.The PGP9.5 positive ganglion cells of expermental group with colonospasm was larger but fewer and the staining was relativelyshallow. Conclusion:The local colon spasms of STC patients has relation with the further reduction of the number and dysplasia of ganglion cellsin the the enteric nervous system.
