我院住院药房退药情况分析及改善措施

摘　　要：目的分析我院住院药房退药情况，探讨减少退药的途径、对策。方法：收集我院2015年1-6月住院临床退药单，根据退药原因、退药药品品种等进行统计与分析。结果：有各种因素导致患者退药用药，原因有不合理用药、患者拒绝用药等，退药品种以抗菌药物为主。结论：严格住院药房退药制度，规范医师的处方行为，加强医患沟通，保障临床用药安全、有效。Objective To analyze the causes of inpatient drug withdrawal of our hospital, and to discuss the method of reduce the phenomenonof drug withdrawal. Methods： Drug withdrawal application since January to June 2015 was analyzed according to the causes, medication types ofdrug return and so on. Results： Various factors lead to drug withdrawal in patients, they are irrational drug use, the patient refused medication,etc. other drug types back to antimicrobial agents based. The main types of returned drugs was antibacterial drug. Conclusion： It is necessary toformulate strict drug withdrawal management system in hospital pharmacy, standardized physician prescribing practices, strengthen the doctor-patient communication, so as to ensure safe and effective medication in clinic.
