门诊药房处方调剂差错分析及应对策略

摘　　要：目的探讨门诊药房处方调剂差错分析及应对策略。方法：收集2014—2015年本院所有门诊处方调剂差错事件。通过调查分析发生出错原因，研究其应对策略。结果：影响处方调剂差错的主要原因为药品摆放不规范、药品质量问题、处方开具错误、药师责任心不强、药品外观相似等。结论：通过加强门诊药房系统管理、提高专业人员的职业素养、促进处方规范化管理，以减少差错，促进临床用药安全。Objective To investigate the causes Analysis and coping Measure for accidents of outpatient pharmacy.Methods：Collection of alloutpatient prescriptions in our hospital during 2014-2015.Through the investigation and analysis of the causes of the occurrence of errors, thestudy of its countermeasures. Results：The main reasons for the impact of prescription dispensing errors were non-standard display, the qualityof medicines, prescriptions issued errors, pharmacists strong sense of responsibility, similar drugs appearance. Conclusion：By strengthening themanagement of outpatient pharmacy system, improve the professional quality of professionals and promoting prescription standardizationmanagement,in order to reduce error and promote the clinical medication safety.
