摘 要:目的探讨高敏C反应蛋白(hsC-RP)对糖尿病周围神经病变(DPN)发病风险的影响。方法随机选取236例T2DM患者,依多伦多神经病变评分系统诊断标准将其分为非DPN组(NDPN)和DPN组,记录两组一般资料及hsC-RP结果,依hsC-RP分布将研究对象分为4组:hsC.RP0.1~0.6mg/L组,hsC-RP0.7~1.4mg/L组,hsC-RP1.5~3.0mg/L组,hsC-RP≥3.1mg/L组,分析不同hsC-RP水平的DPN风险比。结果年龄、糖尿病病程、吸烟史、hsC-RP是DPN的独立危险因素。校正性别、年龄后,hsC-RP≥3.1mg/L组发生DPN风险是hsC-RP0.1~0.6mg/L组的3.4倍;进一步校正BMI、糖尿病病程、吸烟史、HbA1c、血脂的影响后,hsC-RP≥3.1mg/L组的DPN风险较hsGRP0.1~0.6mg/L组升高380%;再进一步校正冠心病、肾病病史的影响后,hsC-RP≥3.1mg/L组DPN风险较hsC-RP0.1~0.6mg/L组升高350%。结论hsC-RP是DPN的独立危险因素;当其≥3.1mg/L时,DPN风险较其≤0.6mg/L时升高。Objective To investigate the correlation of high sensitivity C reactive protein (hsC-RP) with the onset of diabetic peripheral neuropathy (DPN) in patients with type 2 diabetes mellitus (T2DM). Methods 236 patients with T2DM were selected randomly and divided into diabetes without DPN(NDPN group) and with DPN(DPN group) on the basis of the Toronto consensus screening results. General data and hsC-RP were measured. The subjects were also divided into four groups according to hsC-RP values: hsC-RP 0. 1-0. 6 mg/L group,hsC-RP 0. 7~1.4 mg/L group,hsC-RP 1.5~3.0 rng/L group,hsC-RP 3.1 mg/L group. Multivariate logistic regression analysis was conducted to examine the risk factor of DPN and odds ratio (OR) for different hsC-RP levels, Results Logistic regression analysis showed that age, duration of diabetes, cigarette smoking and hsC-RP were independent risk factors for DPN. After adjustment for gender and age,DPN risk in hsC-RP≥3.1 mg/L group was 3.4 times of that in hsC-RP 0.1~ 0.6 mg/L group. After further adjustment for body mass index, duration of diabetes, smoking history, HbA1 c and lipid levels,DPN risk in hsC-RP≥3.1 mg/L'group was 380% higher than that in hsC- RP 0. 1~0. 6 mg/L group. After further adjustment for coronary heart disease and diabetic nephropathy, DPN risk in hsC-RP≥3.1 mg/L group was 350% higher than that in hsC-RP 0. 1~0.6 mg/L.
