不同金双歧治疗方案对慢性乙型重型肝炎患者肠道菌群及血浆内毒素的影响

摘　　要：目的比较金双歧不同治疗方案对慢性乙型重型肝炎患者肠道菌群及血浆内毒素水平的影响效果差异,为慢性乙型重型肝炎的临床诊治和相关研究提供参考依据。方法选取2010年1月-2013年10月医院收治的慢性乙型重型肝炎患者88例分为研究组和对照组,各44例,分别给予金双歧、金双歧联合乳果糖的治疗方案,对两组患者的肠球菌、双歧杆菌、酵母菌和内毒素水平进行观察和比较。结果与治疗前相比,治疗后10d研究组的双歧杆菌显著升高,内毒素水平显著降低;与治疗前相比,治疗后20d研究组的肠球菌、双歧杆菌、酵母菌均显著升高,内毒素水平显著降低;与治疗前相比,治疗后10d对照组的双歧杆菌显著升高,酵母菌和内毒素水平显著降低,与治疗前相比,治疗后20d对照组的肠球菌、酵母菌和内毒素水平显著升高;与治疗后10d相比,治疗后20d对照组的肠球菌、酵母菌和内毒素水平均显著升高,双歧杆菌显著降低。结论单用金双歧治疗方案可对慢性乙型重型肝炎患者肠道菌群进行有效改善,同时能降低患者内毒素水平,采用金双歧联合乳果糖治疗方案对患者内毒素水平降低的效果明显,但用药时间过长可导致患者肠道菌群失调。OBJECTIVE To observe and compare the effects of different Jinshuangqi treatment programs on intestinal flora and plasma endotoxin of patients with chronic severe hepatitis B so as to provide guidance for clinical diagnosis and treatment of chronic severe hepatitis B as well as for related researches.METHODS A total of 88 patients with chronic severe hepatitis B who were treated in the hospital from Jan 2010 to Oct 2013 were enrolled in the study and divided into the study group and the control group,with 44 cases in each group,which were respectively treated with Jinshuangqi and Jinshuangqi combined with lactulose.The levels of Enterococci,Bifidobacteria,yeasts and endotoxin were observed and compared between the two groups of patients.RESULTS The level of Bifidobacteriaof the study group was remarkably elevated after the treatment for 10 days,while the level of endotoxin was remarkably reduced.The levels of Enterococci,Bifidobacteria,and yeasts of the study group were remarkably elevated after the treatment for 20 days,while the level of endotoxin was significantly reduced.The level of Bifidobacteriaof the control group was remarkably elevated after the treatment for 10 days,while the levels of yeasts and endotoxin were significantly reduced.The levels of Enterococci,yeasts and endotoxin of the control group were remarkably elevated after the treatment for 20 days.The levels of Enterococci,yeasts and endotoxin of the control group were elevated more significantly after the treatment for 20 days than after the treatment for10 days,while the level of Bifidobacteria was remarkably reduced.CONCLUSION The single use of Jinshuangqi can effectively improve the intestinal flora of the patients with chronic severe hepatitis B and reduce the level of endotoxin of the patients.The Jinshuangqi combined with lactulose can remarkably reduce the level of endotoxin,however,extended medication time may result in the imbalance of intestinal flora.
