摘 要:目的探讨支气管哮喘(BA)患者气道炎症与肺炎支原体(MP)感染的相关性,为临床控制MP感染、降低BA发病率提供参考。方法选择2013年6月-2015年10月确诊为BA急性发作期150例患者,为BA急性发作期组,同期选择BA缓解期150例患者,为BA缓解期组;比较两组患者MP感染率,测定两组患者中MP阳性者和MP阴性者间的哮喘控制测试(ACT)评分、1秒用力呼吸容积占预计值百分比(FEV1/%)、痰嗜酸性粒细胞(EOS)百分比及血清总免疫球蛋白(IgE),并进行相关性分析。结果 BA急性发作期组患者MP感染64例,感染率42.67%,而BA缓解期组患者感染31例,感染率20.67%,差异有统计学意义(P<0.05);两组MP阳性患者ACT评分和FEV1均明显低于MP阴性患者,痰液EOS和IgE明显高于MP阴性患者,差异均有统计学意义(P<0.05);Pearson相关性分析显示,MP感染率与MP阳性患者ACT评分、FEV1呈负相关性(r=-0.768、-0.742;P=0.041、0.038),与痰液EOS、IgE呈正相关性(r=0.825、0.838;P=0.026、0.031)。结论 MP感染与BA急性发作期和缓解期的气道炎症和哮喘控制密切相关,尤其在BA急性发作期影响较大。OBJECTIVE To explore the correlation between airway inflammation in bronchial asthma(BA)patients and Mycoplasma pneumoniae infection so as to provide guidance for clinical control of M.pneumoniaeinfection and reduction of morbidity of BA.METHODS From Jun 2013 to Oct 2015,totally 150 patients who were conformed with acute exacerbation of BA were enrolled in the study and assigned as the acute exacerbation of BA group,meanwhile,150 patients with paracmasis stage of BA were set as the paracmasis stage of BA group.The incidence of M.pneumoniae infection was compared between the two groups of patients.The asthma control test(ACT)score,percentage of forced expiratory volume in one second(FEV1%),percentage of eosinophil(EOS),and total serum immunoglobulin E(IgE)were determined and compared between the M.pneumoniae-positive patients and the M.pneumoniae-negative patients in the two groups,and the correlative analysis was performed.RESULTS The M.pneumoniae infection occurred in 64 cases in the acute exacerbation of BA group,with the infection rate42.67%,while the M.pneumoniaeinfection occurred in 31 cases in the paracmasis stage of BA group,with the infection rate 20.67%(P〈0.05).Among the two groups of patients,the ACT score and FEV1 of the M.pneumoniae-positive patients were significantly lower than those of the M.pneumoniae-negative patients;the sputum EOS and IgE of the M.pneumoniae-positive patients were significantly higher than those of the M.pneumoniae-negative patients(P〈0.05).The Pearson correlation analysis indicated that the incidence of M.pneumoniae infection was negatively correlated with the ACT score,FEV1 of the M.pneumoniae-positive patients(r=-0.768,-0.742;P=0.041,0.038)and was positively correlated with sputum EOS,IgE(r=0.825,0.838;P=0.026,0.031).CONCLUSIONThe M.pneumoniaeinfection is closely associated with the airway inflammation of the patients with BA in acute exacerbation stage and paracmasis stage as well as control of asthma,especially,it has great influence
