延续性护理干预对糖调节受损患者血糖及认知行为的影响

摘　　要：目的探讨延续性护理对糖调节受损(IGR)患者血糖及认知行为的影响。方法选取内分泌科门诊就诊及体检中心筛查工作中鉴别出的IGR患者80例,随机分为对照组和干预组,各40例,对照组给予常规护理,干预组在对照组基础上进行1年的延续性护理。观察2组患者干预前后空腹血糖(FBG)、餐后2h血糖(2h PG)、糖化血红蛋白(HbA1c)的变化,行为认知情况的变化,及干预1年后疾病转归情况。结果干预组转为糖尿病患者比例为7.5%(3/40)少于对照组的27.5%(11/40),差异有统计学意义(P<0.05)。干预后,干预组FBG、2h PG、HbA1c水平均降低,且低于对照组,差异均有统计学意义(P<0.05或P<0.01);干预组糖尿病相关知识知晓率、合理膳食率、坚持锻炼率和自测血糖率均提高,且高于对照组,差异均有统计学意义(P<0.01)。结论对糖调节受损患者实施延续性护理可以有效改善血糖状况,提高患者疾病行为认知情况,降低糖尿病的发生。Objective To investigate the effect of continuous nursing on blood sugar level and cognitive behavior of patients with imparted glucose regulation （IGR） . Methods A total of 80 IGR patients screened by outpatient examination were randomly divided into the control group and the intervention group, with 40 cases in each group. Both two groups were given routine nursing care and the 1-year continuous nursing intervention was applied to the intervention group. The prognosis of disease was observed 1 year after intervention. The changes in fasting blood glucose （FBG） , 2-hours post-prandial glucose （2h PG） , glycosylated hemoglobin （HbA1c） and cognitive behavior of patients were compared between two groups. Results The proportion of patients with a prognosis of diabetes was lower in the intervention group than that of controls （1.5% vs. 21.5% , P 〈0.05）. The level of FBG, 2h PG and HbA1c were decreased in the intervention group after intervention, and were significant lower than those in the control group （P〈0.05orP〈0.01） . The intervention group achieved a higher rate of disease awareness, rational dietary, maintained exercise and blood sugar self-measure compared with the control group （P 〈0. 01） . Conclusion The application of continuous nursing can improve the effect of blood glucose control and cognitive behavior in patients with imparted glucose regulation.
