摘  要:目的探讨冠状动脉严重狭窄的稳定性心绞痛(SAP)患者循环微小RNA-21(miR-21)与冠状动脉侧支循环(CCC)形成的关系。方法连续入选本院心内科住院的SAP患者130例,患者冠状动脉至少1支血管≥90%狭窄,根据Rentrop分级分为CCC良好组(2~3级)64例和CCC不良组(0~1级)66例,并选同期门诊健康体检者50例为对照组。检测血浆miR-21及血清血管内皮生长因子(VEGF)水平,分析miR-21和VEGF水平与CCC的关系及miR-21与VEGF的相关性。结果 CCC不良组和CCC良好组血浆miR-21及血清VEGF水平均明显高于对照组(P〈0.05,P〈0.01),且CCC良好组显著高于CCC不良组[(0.86±0.10)2-△△Ct vs(0.74±0.10)2-△△Ct,P〈0.01;(336.7±62.8)ng/L vs(286.2±63.3)ng/L,P〈0.01];血浆miR-21的ROC曲线下面积(AUC)为0.833,血清VEGF的AUC为0.844。CCC良好组血浆miR-21与血清VEGF水平呈正相关(r=0.645,P〈0.05)。CCC不良组血浆miR-21与血清VEGF水平无相关性(r=0.146,P〉0.05)。血浆miR-21和血清VEGF水平是SAP患者CCC良好的独立预测因素(P〈0.05)。结论 SAP患者血浆miR-21及血清VEGF水平与CCC形成良好相关,miR-21通过影响VEGF水平表达,可能是促进患者CCC形成的重要机制之一。Objective To study the relationship between circulating miRNA-21 and coronary collateral circulation(CCC)formation in stable angina pectoris(SAP)patients.Methods One hundred and thirty SAP patients admitted to our hospital from January 2012 to December 2014 were divided into good CCC group(n=64)and poor CCC group(n=66)according to their Rentrop grade with 50 subjects undergoing physical examination served as a control group.Their serum miRNA-21 and VEGF levels were measured.The relationship of circulating miRNA-21 and VEGF levels with CCC formation was analyzed.Results The serum miRNA-21 and VEGF levels were significantly higher in good CCC group than in poor CCC group and control group(0.86±0.102-△△Ct vs 0.74±0.102-△△Ct,P〈0.01;336.7±62.8ng/L vs 286.2±63.3ng/L,P〈0.01).The AUC under the ROC for serum miRNA-21 and VEGF levels was 0.833 and 0.844 respectively.The serum miRNA-21 level was positively related with the serum VEGF level in good CCC group(r=0.645,P〈0.05).However,the serum miRNA-21 level was not significantly related with the serum VEGF level in poor CCC group(r=0.146,P〈0.05).The serum miRNA-21 and VEGF levels were an independent predictor for good CCC in SAP patients(P〈0.05).Conclusion The serum miRNA-21 and VEGF levels are closely related with the CCC formation,and miRNA-21 promotes the CCC formation by down-regulating the VEGF expression in SAP patients.
