大柴胡汤配合中药灌肠治疗急性胰腺炎30例

摘　　要：目的:观察大柴胡汤内服配合中药灌肠治疗急性胰腺炎的临床疗效。方法:将60例患者随机分为对照组和治疗组,每组各30例。两组患者均给于胃肠减压,静脉营养支持,补充液体,抑制胰腺外分泌,抑酸,抑制胰酶活性及抗感染等基础治疗。对照组给予生理盐水250 m L加生长抑素3 mg,持续静脉滴注,速度为20 m L·h-1;生理盐水100 m L加兰索拉唑30 mg,静脉滴注,质量分数5%葡萄糖500 m L加加贝酯0.2 g,静脉滴注,视病情轻重运用抗生素。治疗组在对照组的基础上加用大柴胡汤内服及中药大承气汤加减灌肠。观察两组患者的临床疗效。结果:治疗组有效率为93.33%,对照组有效率为73.33%,两组患者临床疗效比较,差异有统计学意义(P<0.05)。结论:大柴胡汤内服配合中药灌肠治疗急性胰腺炎临床疗效显著。Objective： To observe the clinical curative effect of Major Bupleurum Decoction in combination with TCM Enema on acute pancreatitis. Methods： Sixty patients with acute pancreatitis were randomly divided into the control group and the treatment group,with 30 cases in each group. First,the ones in both groups were given basic treatment including gastrointestinal decompression,intravenous nutrition support,fluid supplement,inhibition of pancreatic exocrine,acid suppression and inhibition of pancreatic enzyme activity,and resistance to infection. Then the ones in the control group were added 250 ml of saline ＋ 3 mg of somatostatin for continuous intravenous drip at the speed of 20 ml·h- 1; 100 m L of saline ＋ 30 mg of lansoprazole for intravenous drip; 500 m L of glucose with 5% density ＋ 0. 2 g of gabexate for intravenous drip; Proper antibiotics is used according to degree of the disease. The ones in the treatment group were added Major Bupleurum Decoction for oral administration and Major Purgative Decoction for enema. At last,the clinical curative effects of the two groups were observed. Results： The effective rate of the treatment group was 93. 33%,while that of the control group was 73. 33%; And the difference between them had statistical significance（ P〈0. 05）. Conclusion： Major Bupleurum Decoction for oral administration Major Purgative Decoction for enema in combination with has a remarkable clinical curative effect on acute pancreatitis.
