顶压手法结合多针撬拨复位内固定治疗跟骨骨折65例

摘　　要：目的:观察顶压手法结合多针撬拨复位内固定治疗跟骨骨折的临床疗效。方法:选取本院2011年10月—2013年12月诊治的跟骨骨折患者130例,采用随机数字表法将患者分为对照组和观察组,每组65例。对照组患者给予切开复位内固定治疗,观察组患者给予顶压手法结合多针撬拨复位内固定治疗。结果:观察组患者下床活动时间、负重行走时间、住院时间、住院费用、骨折愈合时间均优于对照组(P<0.05);观察组疼痛评分、功能评分、总评分均明显优于对照组(P<0.05);观察组Bohler角、Gissane角、跟骨丘部高度、单足步态站立相时间、足弓指数、距下关节灵活度、跟骨中部宽度均明显优于对照组(P<0.05);观察组优良率为98.5%,对照组优良率为89.2%,观察组优于对照组(P<0.05);观察组并发症发生率明显低于对照组(P<0.05)。结论:顶压手法结合多针撬拨复位内固定治疗跟骨骨折临床疗效显著。Objective： To observe the clinical effects of topping up and pressing manipulation combined with internal fixation of multiple-needle poking reduction on calcaneal fracture. Methods： Totally 130 cases with calcaneal fracture hospitalized between October,2011 and December,2013 were randomly divided into control group and observation group,with 65 cases in each group. Control group were given open reduction and internal fixation while observation group were performed topping up and pressing manipulation combined with internal fixation of multiple-needle poking reduction. Results： The off-bed activity time,weightbearing walking time,hospitalization duration,hospitalization expense,heating time of the fracture in observation group were better than these in control group（ P〈0. 05）. The pain score,functional score and total score in observation group were better than these in control group（ P〈0. 05）. The Bohler angle,Gissane angle,cumulus height,time of single-foot stance,index of the arch of the foot,flexibility of the subtalar joint and width of the middle part of the calcaneus in observation group were all better than these in control group（ P〈0. 05）. The excellent and effective rate was 98. 5% in observation group and 89. 2%,with statistical significance（ P〈0. 05）. The occurrence rate of complication in observation group was obviously lower than that of control group（ P〈0. 05）. Conclusion： Topping up and pressing manipulation combined with internal fixation of multiple-needle poking reduction has remarkable therapeutic effects on calcaneal fracture.
