摘 要:目的:观察维生素E对大鼠腹壁浅血管皮瓣缺血再灌注损伤的保护作用。方法:利用雌性Wistar大鼠制作腹壁浅血管为蒂的缺血再灌注皮瓣模型并进行试验,将30只Wistar大鼠按随机数字表法分为假手术组、生理盐水组和维生素E组,每组10只。假手术组不做缺血再灌注处理,维生素E组术前1 h腹腔注射维生素E 100 mg/kg,生理盐水组术前1 h腹腔注射等体积生理盐水。生理盐水组和维生素E组于皮瓣缺血2 h再灌注1 h后、假手术组于术后3 h,均取皮瓣中段边缘全层皮瓣组织1 cm×1 cm,检测丙二醛(MDA)含量和超氧化物歧化酶(SOD)活性,光镜下观察病理组织变化。结果:生理盐水组MDA含量明显高于假手术组和维生素E组,比较差异均有统计学意义(P<0.05);生理盐水组SOD活性明显低于假手术组和维生素E,比较差异均有统计学意义(P<0.05);假手术组未见明显组织创伤改变,生理盐水组皮瓣组织损伤较明显,而维生素E组明显好于生理盐水组。结论:维生素E具有抗氧自由基,减轻自由基的脂质过氧化作用,对缓解大鼠皮瓣的缺血再灌注损伤起着一定的保护作用。Objective: To investigate the protective effect of vitamin E on rat flaps with ischemia- reperfusion injury.Method: A total of 30 healthy female Wistar rats were used to establish ischemia-reperfusion injured skin flap along superficial epigastricartery, all rats were randomly divided into the sham-surgery group, vitamin E group and the saline group, each group had 10 rats.The ischemia-reperfusion injury was not induced in the sham-surgery group, an intraperitioneal injection of vitamin E ( 100 mg/kg ) was given in the vitamin E group one hour before operation, saline ( the same volume ) was given in the saline group one hour before operation. Distal tissue was selected from skin flap ( middle edge flap full-thickness flap tissue 1 cm x 1 cm ) in both vitamin E group and saline group 2 hour after ischemia 1 hour after reperfusion, as well as in the sham-surgery group 3 hours after operation to measure superoxide dismutase ( SOD ) and malonaldehyde ( MDA ) contents, pathological tissue changes were observed under light microscope.Result: The MDA content in the saline group were significantly higher than those of the sham-surgery group and vitamin E group, the differences were statistically significant ( P〈0.05 ) .The SOD activity in the saline group were significantly lower than those of the sham-surgery group and vitamin E group, the differences were statistically significant ( P〈0.05 ) .The sham-surgery group had no obvious change of tissue trauma, the saline group flap tissue had more obvious damage, the vitamin E group was significantly better than that of the saline group.Concluslon: Vitamin E can resist lipid peroxidation of free radicals, and play an important protective role in alleviating skin flap ischemia-reperfusion injury.
