摘  要:目的:系统评价患者血小板与冷沉淀联合输注在大出血临床治疗中的效果。方法:计算机检索中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)和万方数据库(Wanfang Date)。采用Rev Man 5.3软件对15篇随机对照试验(RCTs)的1316例患者进行Meta分析。结果:与单用血小板相比,血小板与冷沉淀联合输注组的PT、APTT和平均止血时间均显著短(P<0.01),24 h内悬浮红细胞使用量显著少(P<0.01);止血有效率显著高(P<0.01),输血后的PLT无显著变化(P>0.05);与单用冷沉淀相比,血小板与冷沉淀联合输注组的PT、APTT和平均止血时间均显著短(P<0.01),24 h内悬浮红细胞使用量显著少(P<0.01),输血后的PLT和止血有效率显著高(P<0.01)。结论:血小板与冷沉淀联合输注有效纠正了大出血患者的凝血功能障碍,止血效果更为显著,为挽救患者的生命赢得了宝贵的时间。Objective: To systematically evaluate therapeutic effects of co-infusion of platelets and cryoprecipitation in the treatment of patients with massive blood loss.Method: The domestic lectures were retrieved from databases including CBM, CNKI, Wanfang Database.The meta-analyses were performed with RevMan 5.3 software for 15 randomized controlled trials(RCTs) of 1316 patients.Result: Compared with platelet group, PT, APTT and the average hemostasis time of co-infusion were all significantly shorten(P〈0.01), the consumption of suspended red blood cells in 24 h was decreased significantly(P〈0.01), and hemostatic efficiency was improved significantly(P〈0.01), and post transfusion PLT had no significant changes(P〉0.05).Compared with cryoprecipitation group, PT, APTT and the average hemostasis time of co-infusion were all significantly shorten(P〈0.01), the consumption of suspended red blood cells in 24 h was decreased significantly(P〈0.01), and hemostatic efficiency and post transfusion PLT were improved significantly(P〈0.01).Conclusion: Co-infusion overcome the coagulation disorders in patients with massive blood loss, and it is the best method of hemostatic effect.It will win enough time for saving the patient's precious life.
