回顾性分析超声检查结节性筋膜炎的临床意义

摘　　要：目的：探讨结节性筋膜炎二维及彩色多普勒超声声像图特点,分析超声误诊原因,以提高对结节性筋膜炎的诊断水平。方法：回顾性分析23例经手术和病理证实为结节性筋膜炎患者的术前超声声像图表现,并归纳总结结节性筋膜炎的临床及超声病理特征。结果：23例结节性筋膜炎超声表现为单发梭形、椭圆形结节,或有分叶状;病灶最大径约0.7~3.6 cm,平均（1.0~2.8）cm。14例为低回声,部分夹杂中高回声结构;5例为中等回声;2例为高回声,中央见不规则无回声;2例为低-无回声;边界清楚或欠清楚,无明显包膜,18例可见肿块两端筋膜汇合成线样延伸,即呈“筋膜尾征”;23例结节后方回声均无衰减,19例结节后方回声增强;与皮肤无粘连,对邻近组织无明显影响;CDFI检查：17例结节内部及周边未见明显血流,6例结节内部或周边可测及血流信号,脉冲多普勒显示为低速高阻的动脉血流。超声提示皮下结节15例（65%,15/23）,超声误诊8例（35%,8/23,脂肪瘤3例,积液2例,囊肿1例,转移瘤1例,血管瘤1例）。结论：结节性筋膜炎超声声像图有一定的特征性,超声检查结合病变部位及临床表现对结节性筋膜炎的诊断及鉴别诊断具有重要的临床价值。Objective To investigate the nodular fasciitis of two - dimensional and color Doppler ultrasonographic characteristics, analyze the cause of misdiagnosis of ultrasound,improve the level of diagnosis of nodular faseiitis. Method A retrospective analysis of 23 patients with surgically and pathologically proved nodular fasciitis in patients with preoperative ultrasonographic manifestations, and the clinical and pathological features of ultrasonography of nodular fasciitis were summarized. Results 23 cases of nodular fasciitis ultrasound showed single fusiform,oval or lobulated nodules;maximum lesion diameter of about 0. 7 -3.6 cm, the average 1.0 - 2. 8 cm. 14 cases were low echo ,5 cases were medium echo ,2 cases were medium echo, 19 cases were high echo ,2 cases were low echo,no obvious capsule, 18 cases showed no significant effect on adjacent tissues ,6 cases showed no significant effect on adjacent tissues. 23 cases showed no significant effect on the adjacent tissues. 17 cases had no obvious blood flow. 15 cases of subcutaneous nodule （65 %, 15/23 ） ,8 cases of misdiagnosis （ 35 % ,8/23 ） ,3 cases of tumor,2 cases of cyst, 1 cases of cyst, 1 cases of metastatic tumor, 1 cases of hemangioma,8 cases were diagnosed by ultrasound. Conclusion Nodular fasciitis uhrasonography has certain characteristics ,has important clinical value of uhrasonography and pathological changes and clinical manifestations in diagnosis and differential diagnosis of nodular fasciitis.
