侧卧位硬膜外腔分段注药对连续硬膜外麻醉效果的影响

摘　　要：[目的]探讨侧卧位硬膜外腔分段注射药物对连续性硬膜外麻醉的临床效果。[方法]本院接受治疗的手术患者120例，随机分为两组。观察组侧卧位分段注药，对照组平卧位注药。比较两组麻醉效果。[结果]观察组一次注药后双侧阻滞占6i．67％（37／60），二次注药后占95．00％（57／60），三次注药后占100．00％。对照组依次为41．670（25／60），76．67％（46／60），83．33Yo（50／60）。观察组双侧阻滞率明显高于对照组，且差异均有显著性（P〈0．05）。观察组切皮VAs评分为（1．04±0．62）分，钝性分离VAS评分为（3．28±0．73）分。对照组分别为（1．38±0．64）分和（3．77±0．72）分。观察组VAS评分低于对照组，且差异具有显著性（P〈0．05）。观察组仰卧位综合征占3．33％（2／60），明显低于对照组28．33％（17／60），肌松优良占88．33％（53／60），明显高于对照组63．33％（38／60），且差异具有显著性（P〈0．05）。[结论]侧卧位硬膜外腔分段注射药物对连续性硬膜外麻醉效果好，疼痛评分低，安全性高，临床上值得推广。[Objeetive] To analyze and discuss the clinical effect of lateral position of epidural periodic in- jection on repeat epidural anesthesia. [Methods]A total of 120 patients who underwent surgery in January 2012 to January 2015 at the hospital were selected as the research object and randomly divided into an observation group （ n = 60） and a control group （ n = 60）. Patients in the observation group received epidural periodic in- jections on lateral position, while patients in the control group received injections in supine position. [Results] In the observation group, bilateral blockade occurred at the rate of 61.67%, 95.00% and 100% after the first, second and third injection. In the control group, bilateral blockade occurred rates after the first, second, and third injection were 41.67%, 76.67% and 83.33%. The occurrence rate of bilateral blockade in the observation group was significantly higher than that of the control group; the difference was statistically significant （ P 〈 0.05）. In the observation group, the incision VAS scores were （1.04±6 0.62） points and the blunt dissection VAS score was （3.28 ± 0.73） points, while the incision VAS score and blunt dissection score in the control group were （1.38 ± 0.64） points and （3.77 ± 0.72） points, respectively. The VAS scores in the observation group were lower than in the control group; the differences were statistically significant （ P 〈0.05）. In the observation group, supine syndrome accounted for 3.33%（2/60） of patients, which is significantly less than in the control group, in which it accounted for 28.33% （17/60） of patients. In addition, excellent muscle relaxant accounted for 88.33% （53/60） of patients, which is significantly more than in the control group, in which it accounted for 63.33% （38/60） of patients.
