肾结石ESWL术后肾脏形态学变化及尿石2号方防治临床研究

摘　　要：目的:研究肾结石ESWL术对肾脏损伤形态学变化的影响及中药尿石2号方的防治作用、临床疗效。方法:60例肾结石ESWL术患者随机分为两组,碎石后常规治疗为适当多饮水、抗感染、体位排石和对症处理,观察组ESWL术前3天及术后7天服用尿石2号方。两组分别于ESWL术前、术后24h、72h、168h B超测定肾脏体积、患肾皮质厚度、尿中红细胞数,术后第12天统计结石排石率与结石排净率。结果:ESWL术后24h两组肾脏体积、患肾皮质厚度、尿中红细胞数均明显增加,与ESWL术前比较,P﹤0.01;ESWL术后24h尿中红细胞数比对照组少,P﹤0.05;72h观察组各指标即恢复正常,与对照组比,P﹤0.05;两组ESWL术后168h各指标值均恢复到正常;观察组结石排净率优于对照组,P﹤0.01。结论:肾结石ESWL术可引起肾脏的损伤,但在一定的时间与范围内是可逆的;中药尿石2号方对肾结石ESWL术后肾脏的损伤具有快速、较好的防治作用;中药尿石2号方可提高肾结石ESWL术后结石的排净率,可作为ESWL术后常规用药。Objective：Research on renal calculi ESWL surgery about the effects of morphological changes of kidney and the prevention and treatment of TCM prescription No. 2 urolithiasis. Methods：60 cases of renal calculi patients after ESWL surgery were randomly di-vided into two groups, the control group was under routine treatment including drinking more water, anti infection, posture calculus discharging and symptomatic treatment after surgery. The treatment group was taken No. 2 urolithiasis 3 days before and 7 days after ESWL surgery. The renal volume, cortical thickness and the number of red blood cells were measured in two groups before ESWL, 24 hours, 72 hours and 168 hours after operation respectively. The rate of stone removal was also measured 12 days after ESWL. Results：Compared with preoperative results, the kidney volume, cortical thickness and urine red blood cell count of both two groups were signif-icantly increased 24 hours after ESWL surgery,P〈0. 01. But the urine blood cell count in the treatment group 24 hours after ESWL sur-gery is less than the control group,P〈0. 05. Compared with the control group 72 hours after surgery, the treatment group＇s index values came to normal,P〈0. 05. 168 hours after surgery, both two groups＇ index value were restored to normal. The stone removal rate of the treatment group is higher than the control group,P〈0. 01. Conclusion： Kidney stones ESWL can cause kidney damage, however, the damage is reversible in a certain period time and range. TCM prescription No. 2 urolithiasis has a fast and good effect on the prevention and treatment of the ESWL surgery. The No. 2 urolithiasis can also improve the rate of stone removal after ESWL surgery. And the No. 2 urolithiasis can be used as normal medicine for ESWL surgery.
