摘　　要：目的 :了解我院妇产科超说明书用药情况,促进妇产科安全合理用药。方法 :采用分层随机抽样法,抽取我院2015年11月-2016年2月期间的妇产科门诊处方,参照药品说明书判断是否存在超说明书用药,对超说明书用药类型及各类药品之间的关系进行分析。结果:共抽取妇产科门诊处方26 174张,分析处方用药记录47 113条,涉及用药品种153种。超说明书用药类型主要包括:超给药频次(44.7%),超给药剂量(28.1%),超给药途径(13.3%),超适应证(9.8%)。超说明书用药发生率居前3位的药物种类为:激素类(35.9%),呼吸系统用药(29.3%),造血系统用药(17.4%)。结论:超说明书用药现象在我院妇产科较普遍,医院应重视超说明书用药情况,建立相应管理制度与流程,确保患者用药安全。Objective： To investigate the off-label drug use in our hospital and promote the safe and rational drug use in obstetrics and gynecology. Methods： The prescriptions in department of obstetrics and gynecology of this hospital were randomly selected during Nov. 2015 to Feb. 2016. The behavior of off-label drug use was determined according to the content of drug instruction, and the type of off-label drug use and the relationship of various drugs were analyzed. Results： A total of 26 174 prescriptions for outpatients and 47 ll3 medication records were collected, involving 153 kinds of drugs. The main categories of off-label drug use were off-label dosing frequency （44.7%）, off-label dosage （28.1%）, off-label drug delivery route （13.3%） and off-label indication （9.8%）. The top-three drugs in the list of the incidence of off-label drug use were hormone drugs （35.9%）, drugs for respiratory system （29.3%） and drugs for hematopoietic system （17.4%）. Conclusion： The phenomenon of drug off-label use is popular in department of obstetrics and gynecology in this hospital. The hospital should pay attention to the off-label drug use, and establish the corresponding management system and process to ensure the safety of drug use.
