桂附理中丸治疗不稳定型心绞痛心肾阳虚证临床研究

摘　　要：目的:观察桂附理中丸治疗不稳定型心绞痛(unstable angina pectoris,UAP)心肾阳虚证的疗效以及对血清P选择素(P-selectin,Ps)和游离脂肪酸(free fatty acid,FFA)水平的影响。方法:本研究选择UAP患者共98例,按数字表法随机分为治疗组和对照组,两组均49例。对照组给予常规西药治疗;治疗组在对照组基础上给予桂附理中丸,每次1丸,每天2次,温开水送服。两组疗程均为6周。比较两组中医症状评分,每周心绞痛发作次数以及硝酸甘油片用量;检测两组患者血清Ps和FFA水平。结果:治疗组治疗后中医症状评分显著低于对照组(P<0.01);治疗组有效率为95.92%,对照组有效率为79.59%,治疗组明显高于对照组(P<0.01);治疗后,治疗组每周心绞痛发作次数及硝酸甘油片用量均显著低于对照组(P<0.01);治疗后,治疗组患者血清Ps和FFA水平均明显低于对照组,差异有统计学意义(P<0.01)。结论:桂附理中丸治疗UAP心肾阳虚证可显著改善患者临床症状,减少每周心绞痛发作次数以及硝酸甘油片用量,提高临床疗效,抑制血清Ps和FFA水平可能是其作用机制之一。Objective ：To observe the therapeutic efficacy of Guifu Lizhong Wan in treating unstable angina pectoris （UAP） with syndrome of Yang deficiency of heart and kidney and its influences on serum levels of P-selectin（Ps） and free fatty acid（FFA）. Methods ：98 patients were randomly divided into the control group and the treatment group ,49 cases in each group. The control group accepted routine therapy of Western medicine. On the basis of control group, patients in the treatment group were treated with Guifu Lizhong Wan （one pill per time, bid）. Courses of treatment were 6 weeks in both groups. After treatment, the scores of Chinese clinical symptoms, the UAP attack frequency of one week and dosage of nitroglycerin tablets in the two groups were compared. Serum levels of Ps and FFA were measured in the two groups. Results ： After treatment, the scores of Chinese clinical symptoms in the treatment group were evidently lower than control one（P 〈 0.01 ）. The effective rates of control group and treatment group were 95.92% and 79.59% respectively,the difference was statistically significant（P 〈0. 01 ）. After treatment,the UAP attack frequency of one week and dosage of nitroglycerin tablets for treatment group were obviously lower than control group （ P 〈 0.01 ）. After treatment, serum levels of Ps and FFA in the treatment group were lower than control group with statistical signifieances（ P 〈 0.01 ）. Conclusion ： Guifu Lizhong Wan in treating UAP with syndrome of Yang deficiency of heart and kidney can evidently improve clinical symptoms and clinical effect, reduce the UAP attack frequency and dosage of nitroglycerin tablets. And one of possible mechanisms may be related with suppressing the expressions of serum levels Ps and FFA.
